I had breast implant removal and replacement 9 months ago as my 6 year old implant had ruptured. Last week, I started having similar pains to before (some breast throbbing, shoulder pain, slight numb arm). Been checked by my surgeon who said no signs of capsular contractor or fluid around implant. I asked if it could have ruptured again and he was certain it wouldn’t have at just 9 months after the op. What could be causing this discomfort?

I almost always place implants submuscular. It lowers the rate of capsular contracture significantly. In addition, it looks much more natural because the muscle provides covering over the implant so its not as round on the top. I've also noticed the implants drop less over time...
What you have now is a very large and heavy implant for someone only 5'4". I do not suggest going larger, but going smaller so you have less sagging potential and be more comfortable. Bigger is not always better. You might even need a lift to remove excess skin.
Your implants are probably too heavy for your tissue and have sagged. The inferior pockets can be tightened, but you may need to go smaller to have less weight on the repair.
