More ptosis after breast implant revision with circumareolar lift. What type of breast lift is needed? (photo)

5’1, 100lbs. Underwent breast implant revision with circumareolar lift. I now have more ptosis than I originally had. My nipples are also pointing to the sides instead of straight ahead. Is this something that can be remedied by another doughnut lift, perhaps under local anesthesia?

my preferred lift is a lollipop lift when a periareolar won't do (ie greater than 2cm lift required). The lollipop technique was created by a Canadian plastic surgeon named Elizabeth Hall-Findley and treats the breast as a three dimensional structure instead of two dimensions (anchor - lift). Most importantly it does not require the horizontal component scars that the anchor-lift requires and breasts appear much more natural, lifted, and less boxy. Lastly, the lift lasts longer because the lollipop doesn't rely on the skin to hold the breast up, its actually breast tissue thats being brought together to support the breast and hence improves longevity.

Your implants may be sitting too high, contributing to the appearance of increased ptosis and outward-facing nipples. Before considering another lift, the first step would likely be to lower the implant position (adjust the implant pocket) to align the implants with your natural breast tissue. This can help restore balance and improve overall shape.
Once the implants are repositioned, your surgeon can re-evaluate to see if a periareolar (doughnut) lift is still necessary. In many cases, once the implants sit correctly, the nipple position and overall appearance improve, and a secondary lift may not be needed – or if it is, it can be a much more minor adjustment.
I recommend consulting with your surgeon or a revision specialist to discuss this approach. With the right plan, this can absolutely be corrected, and you can achieve a more natural, balanced look.

No, you have some bottoming out because of the weight of your implants. I would suggest going smaller, tighten the inferior pockets, and do a vertical lift to position your breasts and nipples well. The doughnut lift is limited in what it can do.  These implants are too large and heavy for your frame and tissue.
