Hello! I am interested in hearing what chin alteration options would work best for my face shape. My chin is one of my worst enemies. My side profile always looks kind of crazy like my neck extends straight into my chin/double chin. My weight does fluctuate, but this persists even when I am at my healthiest. The women on one side of my family all have this and it gets exaggerated and worse with age. I am not sure what options would work best for me. How could I best correct this problem?

Based on your pictures, it looks like you could benefit from a chin implant + liposculpture/liposuction your neck and jaw line. Chin augmentation using implants repositions the chin in a forward and outward direction, leading to improved definition of the jawline and neck. The liposuction/liposculpture would contour the area and enhance the results.

A chin that appears small in comparison to the rest of the facial features can contribute to a weak jawline and amplify sagging skin on the neck. With an implant, your chin projection can be increased to create a well-defined contour. Liposuction on the neck removes excess fat which can further enhance your results for a more sculpted appearance.

You are right to evaluate that the contour of the neck is related to hereditary aspects rather than weight or ageing. The volume excess is distributed predominantly deep to the platysma muscles and composed of fatty tissue, digastric muscles and the lower part of the submandibular glands. There is also subcutaneous fatty tissue which can be addressed either through liposuction or direct reduction.

For a comprehensive contour correction, the anterior neck lift tends to offer the most significant impact and has the advantage that it has the tendency to maintain the contour correction for a long time.

In comparison to liposuction however, it does involve a general anaesthetic in most instances and requires placing a scar under the chin which measures approximately 3 cm.

When the volume under the chin is reduced, which you can do also in a computer simulation, the prominence of the chin appears to increase.

Chin Liposuction and Thermitight
Chin liposuction helps reduce the bulge under the chin and can be done for the whole neck. Once the fat is removed the skin can become loose and saggy in some cases. It can be avoided by combining it with Thermitight which is a radiofrequency heat delivered under the skin to tighten it. Both chin liposuction and Thermitight can be done at the same time. The recovery is fast and it can be done under local anesthesia.

In my opinion you are a great candidate for what is known as an anterior neck lift.  This is basically a reshaping of the muscle in the neck through 1 incision under the chin.  This allows direct access to fat, including fat that is under the muscle (which you can't get to with liposuction).  No skin is removed, because at your age the skin can contract and re-drape over the new shape automatically.  I do not think you would have a great result with liposuction alone in my experience.  Through the same incision, a chin implant could be placed as well, although I don't think a chin implant would be as necessary.

Your photos demonstrate several issues going on. You have a recessive Chin profile, so consider placement of the small chin implant to augment your chin forward for better facial balance and proportions, and for better structural support for all the soft tissues in your neck. Liposuction can only accomplish removal of the fat deposits above the platysma muscle, which will only give you minimal results. A surgical neck lift is required to remove the significant fat deposits below the muscle which also includes a platysma plasty to improve the jawline. No skin removal is required. A one inch incision underneath your chin is required for placement of the chin implant and the necklift procedure. The procedures performed under general anesthesia as an outpatient procedure.
