440cc in each breast, do I need a breast lift or smaller implants? (Photos)

At this time, I recommend explantation and lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Later, fat transfers can be performed if additional volume is desired.

If you are unhappy with the size of your breasts, then you can choose to decrease the size of your implants. A breast lift would be recommended to reshape your breasts to make them perkier.

based on your photos, it looks like you would benefit from breast lift. My preferred lift is a lollipop lift when a periareolar won't do (ie greater than 2cm lift required). The lollipop technique was created by a Canadian plastic surgeon named Elizabeth Hall-Findley and treats the breast as a three dimensional structure instead of two dimensions (anchor - lift). Most importantly it does not require the horizontal component scars that the anchor-lift requires and breasts appear much more natural, lifted, and less boxy. Lastly, the lift lasts longer because the lollipop doesn't rely on the skin to hold the breast up, its actually breast tissue thats being brought together to support the breast and hence improves longevity.

Based on your story, it is hard to imagine a board certified plastic surgeon did this. You'll need to see one that is a revision breast specialist to get a great result.

I think you are best served by a full lift. I would also go a bit smaller. It seems your surgeon tried to fill your excess skin with larger implants

You do need a lift, or you will continue to sag. If you want to be smaller, get smaller implants. Smaller will sag less over time.

Based on your before and after photos, your result is acceptable your issue is you need different size implants and downsizing to keep the same size as before surgery. I would recommend an exchange on the larger breast. Do a 300 cc high profile implement on the smaller breast to 325 to 350 C eye profile implant with a full lift. Fees $10,000. You are right in your assessment. You would require a downsizing of your implants along with the wise pattern, master PXi, and probably support as well.
