What are my best options for lip and around-the-mouth hyperpigmentation/melasma? (Photos)

Question

Laser/Fillers/Chemical Peels etc?? I've researched laser treatments. Apparently they aren't so effective for dark skin. Could fillers possibly get me to my desired look? I've used countless bleaching creams to no avail. I've included pics of my face from all angles and lighting as well as pics of my ideal lip/mouth.

You need to find someone that has experience in treating skin type 5.
Combination treatment may be possible.
Filler will not help.
Peels are a possibility.
Remember that there is a risk of the pigmentation getting worse.

For most of our patients, microneedling with some specialized skin care has been shown to be impressive. Microneedling, is a somewhat recent advancement in skin care. You may have heard of many movie stars etc. depending on Microneedling to get their skin in tip top shape. By gently and precisely causing microinjuries in the skin, the skin heals back smoother, healthier, and with more collagen than before a treatment. 
Selective skin care routines coupled with microneedling are effeective at reducing the pigment changes.

I recommend getting a formal evaluation with a cosmetic dermatologist. Melasma needs a combination approach to get the best improvement and will need a series of treatments. In our practice, we combine Clear + brilliant, peels, microneedling/PRP, aerolase, Cosmelan, and sometimes erbium laser (coollaser) to get the best results. Usually there are several different things that we do to help patients with melasma and not one by itself generally gives you the result that you're looking for.  For best results I recommend seeing a qualified provider that has experience in this area.
