A risk of a reverse tummy tuck is the lowering of breasts from tension. This risk is probably increased when combining this with a breast lift. The inframammary folds can be raised with capsulorraphies, but it will be a challenge. Have you discussed this with your original surgeon? This would be the appropriate place to start.

You need scar revision to make the scars look more attractive. At the time of scar revision, the closure can help define the IMF. I would not recommend going to larger implants. You can also revise the tummy tuck at the same time.

Progressive tension sutures are key to preventing a reverse abdominoplasty failure. Your breasts need new implants, capsular tightening with mesh, breast lift scar revision, and a IMF reset via revision reverse abdominoplasty, all of which I've done numerous times.

I am not a fan of the reverse tummy tuck because it often just pulls the breasts down over time. I would suggest repairing the fold, using mesh if needed, but not going larger. A heavier implant will increase your risk of bottoming out again, especially as you age.

Based on your nice post photos, the initial error was doing a reverse tummy tuck at the same time of implants and breast lift now you do not have a capital I capital MF fold formed and have bottom of implants. Best to virtual consult with. But reconstruction of the capital IMF fold is difficult and may take a mesh and suturing technique to the ribs. Fee 10,000 and $20,000 with enlarging the implants also.
