Hello, there are a few options one could explore for vaginal rejuvenation. If you're hoping to address "loose" skin, you may want to look into a labiaplasty which involves the excision of labial tissue and tightening of the labia. If you're looking strictly for tightening, there are surgical as well as non-surgical options available. Be advised that while non-surgical vaginal tightening can help with improving sensation during sex, it won't do much for appearance. It is the result of any one or more of a number of procedures that can make the vagina " more youthful". There are two categories: nonsurgical and surgical. The nonsurgical are radio frequency (eg. Thermiva) and laser (eg. Mona Lisa). Both do the same things: increase collagen and increase blood supply. This is helpful non four condones: stress incontinence, urge incontinence, vaginal dryness , and vaginal stiffness which although does not narrow the vagina, the stiffness can make it feel tighter.The surgical is more complicated, and again, the terms can be confusing. The vagina is connected on six sides:n the entrance or orifice, the apex, the anterior or bladder side, the posterior or rectal side, and the two lateral sides. Each has different issues and different treatment modalities.Your photo indicates defects on the entrance and the posterior side. The rest can not be evaluated by your photo. Your complaint of looseness is what is called "an acquired sensation of smooth/wide vagina". My best analogy is a steel belted tire where the steel belt is torn and you get bulging of the tread which increases the size and smooths out the grooves. There are several ways to approach this but I believe the most solid ,reliable and long lasting technique is called "site specific defect repair". You should definitely find someone knowledgeable with this procedure. IN addition, the entrance or posterior orifice and perineum needs it's own technique as well as the anterior vagina and apex which needs to be evaluated. Yes you can benefit from the appropriate procedures but you need to do your homework and find a surgeon who understands the anatomy and is experienced in the various techniques to give you the best results. Hi! Yes, you are a great candidate for surgical correction. Without an exam, based on the pictures, I would say a vaginoplasty with a perineoplasty would greatly boost the pelvic support, your sex life and your self confidence. However, if you planning another pregnancy anytime soon, you risk destroying the surgical correction. There is also downtime when the muscles are put back appropriately. And having a 2 year old would mean you’d need some help for the first 7-10 days. But it is one of the most satisfactory surgeries amongst my patients. after every pregnancy and vaginal delivery, the vagina undergoes some changes which makes the vagina different than before, so what you are describing is completely normal.
As far as what is available to you, there are surgical and nonsurgical options.

As you are young and most probably will want to have more children I would stay away from any surgical corrections as with future deliveries they will be ruined.

you have none surgical options like using fractionated Co2 laser-like Femi Lift and or Morpheus 8 V microneedling for vaginal tightening and management of mild urinary stress incontinence if these are an issue for you.

These include sagging of the anterior wall of the vagina (aka cystocele), sagging of the posterior wall of the vagina (aka rectocele), gaping of the vaginal opening, and a damaged, shortened perineum. Correcting your looseness issue and improving the appearance to a pre-childbirth state will require a combination of vaginal rejuvenation techniques and pelvic floor repairs.
