My name is Anne, I had a breast augmentation 7 months ago. My implants are MOTIVA 220cc, round smoothsilk, base 9.75, projection 4.0. I really wanted to have some cleavage but even wearing push-up bras I cannot get my breasts to come closer together. My question is if by looking at my pictures can you somewhat predict if the same thing would happen when I get bigger implants. I am planning on getting a second BA with no more than 350cc, hoping the gap will be reduced.
Answer: Motiva implants I think you have near perfect size. I'd keep your implants and instead have the implant pocket revised by placing permanent sutures at the anterior axillary line to close off the side part of the implant pocket and push the implants toward the center. I'd also reinforce this with an internal mesh bra: Galaflex or Durasorb

Answer: Understanding Implant Positioning: Achieving Desired Cleavage After Augmentation Based on the information you've provided, the distance between your breasts is likely influenced by the original implant pocket's dissection. Here are a few considerations regarding your upcoming surgery: Implant Size and Positioning: Increasing the size of your implants to 350cc could potentially fill out the area more, but if the pocket is not adequately dissected medially, the implants may still be positioned widely apart. This can limit your ability to achieve the desired cleavage, even with larger implants. Surgical Technique: To improve the positioning of your implants and achieve better cleavage, your surgeon may need to re-evaluate the surgical technique used during your initial augmentation. Proper dissection of the implant pocket, particularly medially, can help facilitate a closer placement. Consultation with Your Surgeon: It’s essential to have an open discussion with your surgeon about your concerns regarding the gap between your breasts. They can assess your anatomy and determine the best approach to achieve your goals. Consider discussing the possibility of adjusting the pocket during your second augmentation to help bring the implants closer together. Cleavage Goals: Keep in mind that the appearance of cleavage can also depend on factors such as breast tissue coverage, the shape of the implants, and your natural anatomy. Your surgeon can provide insights on how these elements can be optimized in your upcoming procedure. Ultimately, effective communication with your surgeon will be key to achieving the results you desire.

Answer: Can the gap be improved with larger implants? Hi Anne, Wide set breasts depends upon your original chest wall anatomy and breast template. Breast augmentation realistically is not a procedure that aims to alter this balance. It generally enhances what you already have. Larger implants can reduce the distance centrally, but they also will increase the distance laterally possibly creating a less than optimal silhouette. In some cases developing the breast pocket more medially combined with techniques to minimise the lateral extent of the pocket can be used. This however can increase the risk of symmastia (not enough gapping) which should be avoided at all costs. In addition, larger implants may accentuate the natural gap you have meaning that they can fall to the side when lying flat so that the gap is actually increased. A trusted surgeon will be able to identify your individual requirements and give you realistic expectations of what surgery can or cannot be achieved. Using a well fitted bra to create the cleavage you want alongside your augmentation should be a very achievable goal. All the best.

Answer: Gap Improvement with Larger Implant Thank you for a your question. A larger implant can improve the appearance of your gap. However, we do not recommend putting a larger base diameter implant than your chest wall anatomy allows. Many of implant malposition issues are directly related to placing to wide an implant for a patients chest wall measurement. All the best.

Answer: Gap between breasts Yes this gap ,called the inter mammary distance can be reduced to a point, but the width between two breast is somewhat controlled by preexisting anatomy..the width of the sternum or Breast bone and the actual position of the nipple and breast on the chest wall. An implant is somewhat centered behind a nipple .
