Breast lift options? Internal bra? Don't want scarring! (Photo)

You should consider a verticle lift with fat or small implant for shaping.Here are all of the choices:Lollipop lift (vertical lift), donut (periareola) lift, Benelli
lift, crescent lift, anchor lift (inverted T ). Most commonly, we prefer the
lollipop or donut lift because of the shape, small scars and easier healing.
Although the “anchor-shape” or inverted “T” incision is still more popular in
the United States, it represents an older technique with extensive scarring and
a less optimal result in many cases. These newer techniques are utilized in the
vast majority of cases, the rare exception being extremely large breasts. The
nipple-areola complex is repositioned higher, the excess skin is removed, and
the breast is reshaped in a pleasing contour and in a more normal position.

The crescent lift removes a small amount of skin at the top of
the areola. We find that there is very little
lift and the areola becomes oval in shape and distorted with this
method.

The Benelli lift as performed by most surgeons is just the donut
lift, removing a donut shaped piece of areola and skin then using a
#Pursestring or double purse-string suture to tighten the skin. A true Benelli actually will expose much of
the breast through this incision and then the breast itself is shaped and
lifted with sutures. This is a more
complex procedure and is often not done as describe by Dr. Benellil.

The insertion of an
implant as well, may be advisable to restore superior fullness, which is lost
with aging and after pregnancy despite the rest of the breast being overly
large. Fat grafting can be used instead
of implants to improve the contours of the breast.shaping

In order to get the best shape, you are going to need either a vertical lift or an anchor lift. It's not worth going through the procedure if you aren't going to achieve measurable results. Perhaps get a second opinion from another board certified plastic surgeon in your area (link below) and see if he or she agrees. Best, Dr. Nazarian

Thank you for your question.The Horndeski Method (TM) breast lift is another option. Your breasts need to be re-positioned and reshaped aggressively in order to achieve an improved result. A Benelli lift will not be able to accomplish that for you. It is simply not strong enough. You are wise in not wanting the extra risk and complications that breast implants are associated with. The Horndeski Method (TM) has eliminated the vertical scar component that most lifts require and provides upper pole fullness without needing an implant.I hope this helps.Best wishes and kind regards,

A benelli lift would not provide enough of a lift and is best used in conjunction with breast implants. A lollipop or vertical lift would be necessary. A better breast shape is usually worth the additional scar. If your breast volume is different a small reduction on the larger side may be beneficial. Best of luckJosh Olson MD

I feel that you would get the best  result with a vertical mastopexy, which would result in a lollipop scar. This provides optimal shaping and nipple elevation. A Benelli lift does result in less scarring, but does not provide the same degree of tissue and nipple elevation. Also, circumareolar scars from this procedure have more of a tendency to be irregular and potentially spread.

Thanks for your inquiry and picture.  I agree with your surgeon that  you will need a full mastopexy with a vertical component.  You could do the lift first and see how you feel about the fullness and then decide if you want to add an implant in the future.  Please get another opinion if you are not at peace with your options, and good luck. 

Based on the picture included with the question, I would suggest a vertical mastopexy with a slight reduction on the left side for symmetry.  An small implant would greatly assist establishing upper pole fullness which is difficult to create with mastopexy alone. 

You have significant sagging which does not make you an ideal candidate for a circum-areola  lift.However if combined with an internal mesh bra. you may get sufficient lift.If not satisfied you could always proceed wit fat injections or a vertical lift  

Thank you for the photos and though an examination is really needed, I feel from them you are a candidate for a vertical or lollipop lift.  I rarely do the anchor in anyone.Dr Corbin
