I was teased very badly in middle school for the size of the area after I exchanged "nudes" with a boy, he showed all is friends and everyone called me pepperoni..this continued into high school and has affected my self esteem my whole life- I am now 29. I got tattoos around my areolas in an attempt to hate them less I frequently gain/lose 20-30lbs which has made them look even worse What will my tattoos look like after surgery and will gaining/losing weight affect the appearance?

Unfortunately, a large part of the points on your star tattoos will be excised in the areolar reduction. Further, the degree of areolar reduction necessary would disfigure your breast mound shape with a purely circular reduction. You will need a vertical dart in addition to  a circular excision to achieve a predictable areolar diameter with fine scaring and a pleasing breast mound shape. This means a 'lollypop' scar pattern. Opting against that and allowing a more than willing surgeon to do just what you want will lead to breast flattening, thickened scarring, and pleating around the areolar perimeter.

The appearance of your tattoos will be affected by an areolar reduction as much of the tattoo is located in the area that will be removed for the reduction.  You will want to also discuss a lift with your surgeon as It will help improve the overall shape of the breast.  A small implant can be added if you would like more upper pole fullness.
