Witty457I appreciate your concern, and the solution you are inquiring about is a combination of ingredients that provide additional anesthesia and also causes local blood vessels to constrict to decrease bruising. The solution consists of salt water called saline, with the same percentage of salt found in your body solution, 2% lidocaine, for anesthesia and epinephrine, to constrict blood vessels to decrease bleeding and bruising. The solution is injected into the fat below the skin in the areas to be treated and 20 to 30 minutes is allowed to pass to permit the different ingredients to work. The fluid injected before liposuction is called tumescent fluid. It contains epinephrine, lidocaine and sometimes, sodium bicarbonate. This mix helps reduce pain and the bleeding.

The tumescence fluid used in liposuction is Lidocaine, for numbing for comfort and Epinephrine, to minimize blood loss. The fat cells are not actually fragmented with this solution. There are other devices, like ultrasound, that can be used to help fragment the fat prior or with liposuction,

There are only two variables that matter when it comes to liposuction and fat transfer procedures.  These two single variables are equally as important and represent everything patients should be worried about or concerned about.  There’s no possible way you can learn all the nuances of this procedure. You can try but you don’t even have the ability to differentiate what’s important and what’s not.  Please don’t see this as being spoken down to.  The only variables that matter are the candidacy of the patient and who does the procedure.  Some people wash the fat some people don’t. Adding baking soda to the water in the end makes no difference.  Sodium chloride is table salt and all fluids put into the human body should contain some salt.   The variables that matter are so important because they represent the difference from beautiful transformation to grotesque disfigurement.  Choosing the best plastic surgeon is also far more difficult than people believe.  In reality no processing needs to be done of the fat before re-injecting it and doing so has never been proven to have any advantages other than marketing spin.  Best, Mats Hagstrom MD
