Implants not dropping, sagging breasts and flex distortion 10 weeks post op. Will this possibly resolve over time? (photo)

Your plastic surgeon will be the best resource for your concern as he know exactly what was done. At 10 weeks there is still a chance the implants will come down to fill the lower poles of the breasts. If you were my patient I would recommend good frequent breast massage and wear a top band. 

Your plastic surgeon would be able to judge if you need a revision. I usually wait at least 6-12 months to allow implants to settle.  If they have not settled then, you may need something done to make them look better.  Going above the muscle or dual plane is beneficial for patients with your breast shape.  I think doing that will help them look more natural.

You are just 10 weeks post-op so there is a possibility that massaging and wearing a breast strap may help push the implants down.  I would discuss this option with your surgeon as something to try prior to a revision procedure.  If it doesn't improve over the next few months, that would be the best time to discuss your surgical options.  ac

If decide to revise just lower the inferior pockets do not change from unders to overs in my over the internet opinion...

You have several issues going on. Regarding your implants not dropping, at 10 weeks the chance of them dropping much is slim, as the pocket if fairly well formed. It is rare for them to drop much after 12-16 weeks. You will most likely need to have them lowered, but try aggressively massaging them, can't hurt. I do not recommend a completely new pocket above the muscle. Regarding your sagging breast, a lower implant will help that some, but your breast tissue and nipples are still going to be fairly low, unless you  get some type of lift. Regarding your flex distortion, that is hard to avoid when the implants are submuscular.  Regarding whether your breasts will look natural, that depends on what you call natural. I always recommend patients show their surgeon a picture of what they want to look like, and not use words to try to describe the results they want. 

 Your plastic surgeon will always be your best resource given that he knows exactly how/where the breast implant “pockets” were dissected. Generally speaking, at your stage, there is still time for the breast implants to “settle”; this process may be assisted with the use of downward displacement exercises/massage and/or the use of upper breast pole straps. See what your plastic surgeon recommends. Hopefully, as the breast implants “settle”, the animation/flexion “deformity” and overall breast shape will improve. Please discuss with your plastic surgeon about massaging the breast and wearing a wrap to the top of your breasts to help your implants drop.  This will help you have more natural appearing breasts as the implants settle further.

Thank you for your question and photographs.  I am sorry that you are experiencing this difficulty.You are still very early after surgery and your implants may continue to drop further.  Please discuss with your plastic surgeon aggressive breast implant displacement exercises and I personally think a superior pole wraps may still help you.Your plastic surgeon is in the best position to estimate whether a revision may be necessary.  If the pectoralis muscle is still attached to the fifth rib then further dropping of the implant may be difficult.Whether a revision would involve release of the pectoralis muscle or a bilateral breast lift is best determined by your plastic surgeon.  I would however try a pole wrap.
