I have a large butt that protrudes many inches and no clothing fits me. I'm relatively skinny but this is a huge problem my entire life. I had a consultation with a TOP liposuction provider, and they said it was mainly muscle therefore the liposuction will be ineffective. As you can imagine I was devastated. I've yet to meet the surgeon but after receiving this information, I poked and tensed my gluteal muscles, it really does feel like they said. Thanks if you can help alleviate some concerns.

It is very difficult to determine the exact size and shape implant you will require to best match your ideal butt image without an examination by a board certified plastic surgeon. Not just any board certified plastic surgeon, but one with many years of frequently performing breast augmentation surgery including different approaches, techniques and implant choices. This is because several measurements not to mention your breast characteristics are needed to determine the optimal implant size to obtain your goals. Without knowing these dimensions it would be difficult to make this determination. For example, the existing base width of your buttock will determine, in many cases, the maximal volume per implant profile that you can accommodate. Liposuction of your lower back and fat transfer is also helpful as an adjunct to implants. Using “want to be” photos however are useful if simply provided to the surgeon.

Answer: Men with large gluteal muscles The most definitive test would be to have an ultrasound measuring the thickness of subcutaneous fat in your gluteal region. I suppose someone could have a partial gluteus maximus removal or reduction though I have never heard of this procedure nor would I recommend it. Everyone has some layers of fat in this area the question is how much. While pinching it can give you some idea and ultrasound would give you an objective actual thickness of subcutaneous fat telling you approximately how much reduction could be achieved. Ultrasounds are risk free, not particularly expensive and can be ordered by any physician. Best, Mats Hagstrom M.D.
