It appears as though your asymmetry is not only of the gluteal fold but also lacking volume and length on the left side. In my 21 years experience with buttock enhancement/reconstructive surgery, about the only reliable and permanent way to have this much volume and projection added is with a buttock implant, not fat transfer. Once the right size and shape buttock implant is placed, any remaining asymmetries for the gluteal fold may or may not need to be addressed. Please keep in mind that an extremely few number of board-certified plastic surgeons specialize in buttock implants so you may have to travel far and away from home to seek consultation with this particular type of specialist. it is hard to tell for sure without an examination. Based on your photos, it looks like you would benefit from fat transfer to correct indented scar. If you are considering surgery, I would suggest you consult a board-certified plastic surgeon. Only after a thorough examination, you will get more information and recommendations.

I believe your gluteal asymmetry is a congenital defect and happened during embryological development. Most likely the cyst came from that abnormality rather than the other way around.

Creating symmetry is going to be difficult because the left side one most likely not project correctly with simply adding volume with fat grafting.

You’ve included a proposed description on how to correct the problem. I personally would not try to create a gluteal fold because doing so so often and supple looking unnatural. also, your right side does not have a distinct fold so I don’t see the need to create one for the left either.

Whether the right side should be reduced to some degree to help create better symmetry is a point of discussion.

It’s going to be difficult to create perfect symmetry and limited improvements are probably where you should set your goals if moving forward with surgery.

I would continue having consultations until you feel you’ve met the most experienced and talented provider in your community for BBL procedures.
