What is the best option for hypertrophic scars two years after breast augmentation? (Photo)

I guess you get the idea from all the answers that there is simply not a perfect answer to the dilemma of hypertrophic scars.  It is as frustrating for us as it is to you.  Why is one part of the scar flat and the other part raised?  We don't know the answer.I am going to agree with the majority of answers here.  I would try scar revision and be agressive in the postop period with treatement such as the silicone gel sheets or the embrace scar therapy during the healing phase.  I added a link to their webpage.  There are no guarantees with scar treatments but I think revision is worth trying.  Your surgeon has followed you more closely than any of us.  Take the advice of your surgeon seriously as well.

It seems that everything has been done for your to have a good scar.  Unfortunately in some people hypertrophy or keloid results.  You may consider having a scar revision  to improve the scar, by excising and reclosing the scar. 

You will do well with scar revision.  Excise the old scar and the new scar should be better because there will be less tension then immediately after a breast augmentation.Best Wishes,Nana Mizuguchi

There is no easy way to treat hypertrophic scars. From your pictures it appears there is still significant lumpiness as well as redness. I would expect this to improve with more regular and frequent steroid injection combined with silicone tape or cream. Laser may help with the redness but unlikely to reduce the lumpiness of the scar.

Having said that, you could consider some scar treatments from prescription
ointments, silicone sheeting, taping the incisions with steri-strips, or
steroid tape. All of these have been shown to be beneficial. At your situation, you could try some steroid tapes or may require revision surgery.

Hi, al256.  If the hyperpigmentation bothers you, it can be improved with laser.  However, it will not affect/improve the raised scar.  You would benefit from scar revision.  Best regards.

This long after surgery I would recommend excision of the old scars.  This can usually be done under local anesthesia.  Taping of the scar for a longer period of time is necessary.  The closure will not be as tight and you should have a better scar.Best of luck.

You should consider excision of the scars with an eight week postoperative course of embrace advanced scar therapy.  This is a tension loaded adhesive silicone gel sheet that takes tension off the healing scar to reduce or eliminate the inflammatory phase of wound healing leaving a flatter, narrower and fainter scar.  You can look up the embrace product on-line and youtube.
