I'm 5'3" and have a fit body type/slim. I went from a 32A to a 32B/C by having 290cc/grams (silicone implants) 4-5 years ago.Currently I'm using a 32C size bra.The implants that i have are the low/medium profile. My surgeon told me that there wasn't room for more than 345cc/grams of the medium/high profile (not the highest/rounded ones, but the ones just under those) Cohesive Gel implants. Will 345cc/grams help me achieve a D/full D or at least be a good visible dif ?

It is very important to communicate your size goals with your surgeon. In my practice, the use of photographs of “goal” pictures (and breasts that are too big or too small) is very helpful. I have found that the use of words such as “natural” or “C cup” or "fake looking" means different things to different people and therefore prove unhelpful.
Also, as you know, cup size varies depending on who makes the bra; therefore, discussing desired cup size may also be inaccurate.
I use intraoperative sizers and place the patient in the upright position to evaluate breast size. Use of these sizers also allow me to select the press implant profile (low, moderate, moderate plus, high-profile) that would most likely achieve the patient's goals. The patient's goal pictures are hanging on the wall, and allow for direct comparison.
I have found that this system is very helpful in improving the chances of achieving the patient's goals as consistently as possible.
By the way, the most common regret after this operation, is “I wish I was bigger”.

Given your height and body type (slim) and your desire to go larger, I would agree that you should consider a high or moderately high profile implant. However, you should think seriously about whether it is worth it to undergo a second surgery for a 55 cc increase in implant size. It is likely that this won't get you to a D cup. Of course, a determination of how large of an implant you could accommodate would require an examination. Best of luck.

There is no specific cc or volume that will get you to a given cup size. 1- the final result is dependent on your starting point (and everyone has a different starting size or look), 2- One can not have a definite cup size after augmentation because each brassiere maker is different, that is, a "d" cup with one maker may be different from another.

Answer: You will need bigger implants If you had attached picture would have been very helpful. In general when you want to get bigger implants it should be at least 75-100 cc bigger than the implants you already have.

Only an examination followed by intraoperative determination will yield the right answer. What you would need is a high or super-high profile implant that will provide you with significantly more projection without the increased width. The ultimate implant selected would depend on the general appearance that you desire.
If your doctor uses Allergan implants, I would consider either the Style 20 (high profile) or Style 45 (super high profile) silicone ones - and let him/her have final discretion based on your desires.
