Is my end goal achievable for a thick skinned, ethnic nose? (photos)

Question

I was wondering if my after photo (edited) would be realistic to achieve? I have thick skin and a large hump and tip, so I'm not sure whether reducing the tip size, hump and narrowing it from the front would be possible.

Based on your photos, you appear to have a wide nasal appearance with a bulbous droopy tip and a dorsal hump. Please consider consulting with several reputable rhinoplasty specialists who offer computer imaging so you can see what may be achieved after surgery. Hope this helps! Dr Joseph

Thank you for your question and photos. Though thicker skin can present certain challenges when performing a rhinoplasty, creating a desirable result is still achievable. Thick skin on the end of the nose can limit the definition of the tip and contribute to a bulbous appearance. 
In addition to standard rhinoplasty approaches, such as reshaping and resizing the nose, the nasal skin may need to be carefully thinned. In some cases, bone and cartilage grafts may be required to promote a favorable nasal projection. I recommend scheduling an in-person consultation with an experienced, board-certified facial plastic surgeon who can assess your concerns and develop the best approach to help you achieve your aesthetic goals.

Hi. Of course, your nose can become even more delicate. However, you should definitely know that this has its limits because it has thick skin. I would like you to know that the reason why your nose looks even bigger than it is is your very small chin. Nose surgery is of course a preference, you can try for a little better, but if you do not have chin surgery, nose surgery will not be of much benefit. The important thing to keep in mind is that when performing rhinoplasty on thick-skinned patients, the nose cannot be overly shortened or reduced- otherwise it will loose shape and definition. I typically recommend maintaining tip projection in these patients to keep the skin well-fitted around the underlying nasal structures. I think the edited photo you made is a reasonable goal given that you did not overly deproject your nose.

Your simulation is realist and possibly still conservative with regards to the degree of improvement. Although your skin is thick, your tip cartilage is prominent and can be reshaped and supported well using structural techniques.

Thank you for your question. I think rhinoplasty is one of the most beautifying surgeries that we do as plastic surgeons. A large portion of my practice is dedicated to rhinoplasty surgery.  In your case, your morphed image is realistic.  I would also strongly recommend a chin implant as well as fat transplantation into the labial mental groove.  This is the area between your chin and your lip that indents.  A chin implant will make that worse unless you place fat in this area.  It is important to only see plastic surgeons with extensive experience in rhinoplasty, ie perform at least 6 per month and have performed hundreds of cases. Also, critically evaluate on-line galleries to see if their art form resonates with yours. Rhinoplasty is definitely a grey haired plastic surgeon's sport. Best wishes to you.

I think you can end up with a result even nicer than the one you show. I would suggest going on a few consult. Bear in mind that many plastic surgeons are very conservative when it comes to rhinoplasty. You need to find one who will make the dramatic change you want and need.

Based on your photographs, it would be possible to achieve the hump reduction pictured.  It is hard to tell how much narrowing and tip refinement you want from a side view, but that is also possible at time of rhinoplasty.  I do a significant amount of ethnic rhinoplasty in my practice, and your concerns are very common in my patients.  Dealing with the thick skin would involve a combination of removing any excess tissue and ensuring strong cartilage support for the modifications needed.  I would recommend consultation with a board certified plastic surgeon with experience in ethnic rhinoplasties.
