I was wondering if my after photo (edited) would be realistic to achieve? I have thick skin and a large hump and tip, so I'm not sure whether reducing the tip size, hump and narrowing it from the front would be possible.

Though thicker skin can present certain challenges when performing a rhinoplasty, creating a desirable result is still achievable. Thick skin on the end of the nose can limit the definition of the tip and contribute to a bulbous appearance.

Answer: Rhinoplasty and thick skin outcomes The important thing to keep in mind is that when performing rhinoplasty on thick-skinned patients, the nose cannot be overly shortened or reduced- otherwise it will loose shape and definition. I typically recommend maintaining tip projection in these patients to keep the skin well-fitted around the underlying nasal structures. I think the edited photo you made is a reasonable goal given that you did not overly deproject your nose.
