Am I a candidate for butt implants plus BBL? (Photos)

Question

Hi, i am 55 years old and always had a dream to have perfect butt. Now, i am ready to get started. However, i have worries , i have heard that the butt implants can move or flip over... Please, help me to make right decision and find a doctor. Thank you.

In short, butt implants yes, BBL no and not necessary. Please read on.

Allow me to answer your question as best i can based upon your photos. However, please keep in mind that an in-person, or at least a virtual, consultation with physical exam is first necessary before any legitimate evaluation or final surgical recommendations can be made.

There are currently only two FDA approved surgical options for augmenting the buttock/hip area:  1) Buttock and cosmetic Hip Implants (which are soft semi-solid silicone rubber implants that cannot rupture &/or leak) and 2)Brazilian Butt Lift (aka BBL) - which utilizes the patients own fat from liposuction then transfers this fat into the buttock. On the other hand, BBL using Sculptra is a dangerous waste of money and is NOT FDA approved. and/or hips and   . So what it comes down to, like any surgery, is proper patient selection and long-term results. Implants provide the only guarantee for a sustained permanent result and tend to be ideal for perking up not only lean or deflated buttocks but also providing substantial projection to any butt, which is what every single patient wants.

Unfortunately, fat typically melts away leaving disappointing results after 12 months, often sooner.  Unless your BBL surgeon is injecting only 200 to 300 ccs of fat into each buttock cheek, a large percentage of that fat is going to die.  Dead fat leads to dents and dimpling like cellulite, unevenness, loss of projection, and fatty necrotic oil cysts which are hard marbles and can be very tender 
Unfortunately, because ~70+ % of the fat transferred will melt away within a year, most patients are not good candidates for BBL because they lack an adequate amount of fat to harvest. Beware, if a surgeon asks you to purposely gain weight (i.e. fat) before BBL, know that the fat you lose first as you get back to your baseline weight after surgery is that very same fat that was transferred into your buttock, hence your new buttock shrinks first, the fastest, and the most...so please do not fall victim to this recommendation. Even those patients that had adequate amounts of fat pre-operatively, still end up seeking buttock implants after a year or so because most of the fat transferred melted away leaving them with minimal result.  Fillers such as Sculptra are cost prohibitive, provided enough volume is injected (i.e. at least 400 cc of active ingredient costing over $300,000) and last only 12 - 18 months.  So, if you're looking for a reliable, predictable, and long-term permanent option that also provides projection in the central buttock, buttock and hip implants are your solution. Please note that an extremely few number of board certified plastic surgeons specialize in buttock implants, so you may need to travel far and away from home to find this particular surgeon.  If you do have surgery by a true specialist, your risk of implant problems such as flipping or moving should be less than 0.1%.  I hope this is useful information. Glad to help.

Based on these photos, you should also consider fat grafting of buttocks. Considering volume you need for shaping/augmentation, one can expect that sufficient amount of fat will be available after liposuction of torso (lipo 360). Liposuction will further improve your abdomen and waist and fat augmentation will give desired size/shape. In person consultation with a board certified plastic surgeon, with experience in both techniques, is recommended. Good luck.

Thank you for your question.  You are an excellent candidate for a Brazilian butt lift with a small buttock implant.  The implant adds a lot in terms of projection and shape to the buttock while the fat transplantation helps prevent being able to feel the implant and gives width to the buttock.  Performing body contouring-liposuction of the hips would further improve the appearance of your buttock.  Best wishes to you.

Great posted photos. I recommend a "Skinny BBL" Or "SKINNY COMPOSITE BBL" first. the first is just lipo with fat transfer; the second is a combination of lipo and injections of Sculptra/Radeisse with fat injected. Both must be under ultrasound guided injections by Florida office surgery rules. Fees from $7,900-$15,000+. As for Gluteal implants they have very high infection rates and yes flipping over rates. In regards to candidacy for gluteal implants you are actually a good candidate and that your backside has very little projection. The problem with gluteal implants is that the procedure has a higher complication, failure and rate of dissatisfaction. Most plastic surgeons don’t offer the procedure and have no experience in doing it.
Considering how complex and how high the complication rates are if you’re going to go for implants it should be done by someone who specializes in doing gluteal implants which there aren’t that many. implants need to be placed intramuscularly and the size needs to be contained so that it is properly fits within the borders of the gluteus muscles. A true intramuscular implant placement will prevent the implant from moving, flipping and showing the edges when you move in certain positions. To Date I have answered over 14,000 patient questions on this website. most of them are about Liposuction and a third transfer since that’s what I do most. I also answer questions on a glutio augmentation so implant questions do come up. I don’t think I’ve ever had a post or someone was happy with their gluteal implants. Usually the posts are about the implants getting infected or patients being unhappy with them because the edges show or they have flipped or changed locations.
For implants it is extremely important you find one of the few doctors who truly does intramuscular implant placement and that you are willing to put up with the high complication rate which includes potentially disfiguring complications.
I can speak to the BBL better since I do lots of them.
Personally I would not do the two procedures at the same time. my recommendation would be to do the BBL first and if the results are in sufficient then consider gluteal implants. it can be challenging to get a lot of projection on the back and lower back of the buttocks. Women who have curvy bodies do so primarily because of their skeletal structure not because of their fat distribution. Aggressive fat manipulation does not and cannot give her the same appearance as someone who was born with a highly feminine curvy body.
The BBL can improve S-curves and there’s an excellent adjuvant to Liposuction of the torso with creating overall better curves.
While there’s no shortage of providers offering the BBL the difference in outcome depending on who does the procedure varies massively.
Delivering consistent quality results for both Liposuction and fat transfer is more difficult than most people believe.
To find the most talented and experience provider I suggest having multiple in person consultations. During consultations as each provider to show their entire collection of before and after pictures. simply seeing a handful of pre-selected pictures which most likely represent the best results of each providers career is insufficient to get a clear understanding of what average results look like in the hands of that provider.
This is why you should push providers to show you their entire before and after picture portfolio for the procedure you’re interested in. If they’re not willing to do that then ask them to show you as many as possible. an experienced surgeon should have no difficulty showing 50+ sets of before and after pictures at a minimum. Highly experienced providers will have hundreds or even 1000s.
When reviewing pictures always confirm that after pictures were taken at least three months from the date of surgery.
For fat transfer early postoperative pictures can look very impressive but do not at all represent final results.
Anyone showing early results should be immediately discounted and checked off the list as a potential surgeon for your procedure. it can be tempting to include those pictures but showing early fed transfer results is doing a great disservice to patients and the plastic surgery community. it sets expectations extremely high from of what possible for the procedure.
For example pictures taken from the operating room or with patients who still have surgical markings or clear indication of early postoperative after pictures which do not represent what the results will look like long-term.
When reviewing pictures ask the providers to point out exactly what areas were included and what areas were not included in the treatment and have them point out transition zones between treated and on treated areas. look for areas of contour irregularities, divots, streaks or other areas of imperfection on any of the areas treated with Liposuction.
A good outcome should have a patient with a smooth continuous S-curve on their backside that goes from their armpit to their knees.
Different doctors also have very different perceptions of how this procedure should be performed.
There seems to be a trend of doctors purposefully doing liposuction of the outer thigh in order to make the buttocks look larger. this tends to destroy any hope for having a perfect S-curve and creates what I call the “lollipop butt” look. It is undesirable, unnatural and unfortunately different difficult to correct with revision surgery.
A good BBL should simply show perfect feminine curves of the entire torso down to the knees.
I also recommend reading all reviews on various physician review websites avoiding providers to any significant number of justified negative reviews.
The absence of justified negative reviews is more important and more telling than having an abundance of positive reviews.
When in doubt continue having more consultations until you feel quite comfortable that you found the most talented and experienced provider for the procedures you’re interested in.
All of these procedures will change the body permanently and many of them irreversibly.
To minimize ending up with a regretful outcome or disfigurement patients oh it to themselves to put a substantial effort into finding the most talented and experienced provider. Patients often try to do this using Internet searches and comparing websites but in my opinion there is no source of nutrients formation available on the Internet to patients and the only way to really find the best provider requires having a substantial number of in person consultations.
