Fractional laser is an amazing and highly effective treatment in our office. We use the laser treatment to help with acne scars, large pores, fine wrinkles, tissue collagen loss, scars, sebaceous hyperplasia, active acne, and for brightening dark pigmentation. Although there are very strong fractional lasers such as CO2 laser, we use lighter treatments for corrective skincare and maintenance of results. Our Thulium fractional laser, clear and brilliant laser, and Ultra Thulium laser are excellent options in our clinic. We also perform CO2 fractional and erbium depending on skin tone and tolerance for downtime. Patients do have to use a Melarase cream before and after treatment to help further reduce pigmentation after laser. Dark spots can improve with these treatments. Age27 is pretty young to be considering facial surgery due to aging, especiallysince the main reasons for a facelift and other surgeries, like skin laxity orloss of muscle tightness, really aren’t that prevalent yet. Surgery is reserved for people that have excess skin. Without pics I can not make any definitive statement, but in general I would recommend the following:(1) stop smoking (2) good skin care regimen to maintain (cleanser, moisturizer, and sunscreen) (3) a good medium depth peel. This would be a good start but I would see a dermatologist or plastic surgeon (who has an interest in skin care) in you area for a custom tailored plan.

A realistic expectation is important so you are not discouraged. You may have a dramatic improvement, but you first will need to have your health in order, then see a Board Certified Dermatologist or Board Certified Plastic Surgeon, and go over all of your skin options. I agree with cessation of smoking as it causes oxidative injury to the proteins (collagen and elastin) that give your skin resiliency, elaasticity and stiffness. I recommend a retinoid repair cream to be used on a daily basis and suggest RR Retinoid Repair by Kare Skin. Use sunscreen and a moisture booster that is ceramide based like Elevase.
If you look older than your age after years of smoking and drinking, it is unlikely you can re-achieve your teenage youthful look even with surgery. You may be able to improve the quality of your skin with medical skin treatments. A good first start would be to stop smoking and drinking. Consult with a qualified doctor who can explain your nonsurgical options first.

Smoking and significant alcohol consumption, along withother environmental factors including sun exposure, can hasten the appearanceof aging. Quit smoking and reduce your alcohol intake. Good, firstline treatment include topical therapies, including tretinoin, which improvethe texture and tone of your skin with nothing more than a cream. You mayconsider the use of botulinum toxin (Botox) to treat frown lines and crow’sfeet. Additionally, dermal fillers and volumizers may provide improvementwithout surgery. Microdermabrasion and mild chemical peeling may help,but more aggressive chemical peeling and/or laser resurfacing may be inorder. Speak with your Plastic Surgeon regarding your concerns. Ultimately, you should look refreshed and more youthful, but unfortunately,looking 16 again may be unrealistic.
Surgery is generally only performed when one's skin has sagged and is laxed. Even most thirty year olds don't yet have this issue. Certainly without photographs, one could not make a final assessment, however, perhaps you may want to look at other skin rejuvenating procedures; such as microdermabrasion, photofacials and a good medical skin care system. These will definitely help improve the look of worn or tired skin that can make one appear older than they are.
