Breast Implant Revision for correction of Double Bubble Deformity

This 30 year old mother of 2 has been pregnant 4 times. She is 5'6 tall and weighs 125 pounds. She had a breast lift 6 years previously and a breast augmentation 4 years prior. Her implants at that time were: round, textured, 385cc high profile, regular cohesive silicone implants. She had a very nice result but presented requesting to be bigger, (as big as she can be), new implants perkier, (placed up and in), and also try to have less visible rippling. These requests are all very challenging because she is thin, does not have much soft tissue to hide the implants, and is small framed. We decided to use ultra high profile, smooth, highly cohesive silicone gel implants 700cc placed below the muscle. Her post operative photos are from two years after, showing how the result has been maintained.

Patient had her initial Augmentation 18 years ago with a different physician.  She wanted Dr. Eric Schaffer to remove  her old saline implants  and replace them with larger ones to achieve more fullness and to better match her body shape.  The original implants were filled to 350 cc's and were replaced with Mentor smooth round moderate plus  implants filled to 540 cc's using her original incision site.

Breast Implant Revision to  Remove and Replace Saline Implants

Our patient is a 56-year-old Middle Eastern female who weighed 139 lbs and came to Dr. Timothy Katzen two years after her original breast augmentation because her right breast began to feel tight and look less natural from a thickened scar tissue capsule around the implant. Dr. Katzen recommended a right breast capsulectomy, a breast implant revision procedure that removes the hardened capsule to allow the breast to soften, move more naturally, and regain better symmetry. Under general anesthesia, he reopened the prior incision so scar placement remained as discreet as possible, then carefully separated and removed the abnormal capsule from around the implant. This capsule can act like a shrink wrap layer, changing breast shape and making the implant feel firm. Once the restrictive scar tissue was removed, Dr. Katzen refined the implant pocket, confirmed that the implant was properly seated, and shaped the surrounding tissue so that the breast contour appeared smooth rather than pulled. The tissues were closed in multiple layers to reduce tension, support healing, and improve scar quality. After surgery, a supportive bra and activity guidance help swelling settle and protect the repair while the breast softens over time. These photos were taken 2 months after surgery, and her results are remarkable, showing a softer-looking right breast, improved balance between sides, and a more natural breast augmentation shape as healing continues.

Targeted Right Capsulectomy to Restore Softness in Breast Augmentation Revision

A softer, more natural breast shape is often possible after implant revision surgery when painful capsular contracture is properly treated. Our patient is a 44-year-old Asian female who previously had bilateral breast augmentation and later developed bilateral painful breasts because of capsular contracture around her saline implants. She came to Dr. J. Timothy Katzen seeking the removal of the bilateral saline breast implants, bilateral capsulectomies, and replacing them with silicone implants. Under general anesthesia, Dr. Katzen performed this breast revision procedure by removing the old implants, surgically excising the scar tissue capsules surrounding them, and placing new silicone breast implants to improve comfort, contour, and softness. With breast implant surgeries, capsular contracture happens when the natural scar tissue around an implant becomes tight and constricting, which can cause pain, firmness, and distortion. A capsulectomy is performed to remove that problematic scar tissue so the breasts can be reconstructed more healthily. Silicone implants were chosen to provide a smoother, more natural-looking, and natural-feeling result. Her photos were taken 1 month after surgery and already show remarkable early improvement in breast shape, symmetry, and softness. She is also relieved of most of her pain, making her outcome a beautiful example of how revision breast augmentation can improve both function and appearance.
