19 days post asian eyelid surgery and epicantoplasty. Epi scar has become raised and white.

Question

What's the best way to remedy? Started massaging incision scar with castor oil and silicon taping epi scar 2 days ago. Is this ok? Whats the best way to prevent hypertrophic scar? Should I seek steroid injection or stick with more natural methods for now?

It's common for the medial epicanthoplasty scars to be raised and unsightly in the first few months. With proper scar management, they usually improve after 3-4 months.   I tell my patients to avoid salty/ spicy food and start massaging the scar with silicone scar gel 2-3 weeks after surgery.  I typically do not recommend castor oil/ taping because it can be irritating and taping in that area can be difficult.  Some patients with very thick scars at 1 month post op may benefit from kenalog injection, and some patients may need 2-3 injections one month apart.  Best wishes!Goretti H. I've found that oils in what ever form can irritate eyelid skin.  I think the silicon tape can do you some good.  Scar massage can be okay but as long as you're avoiding salt, seafood, alcohol, and skin irritants it should get better.  Kenalog or other steroid injections can help but only where there is very hard scar tissue underneath the skin.   Redness isn't scar tissue exactly, it's new blood vessels and those go away with time.  I've never actually seen hypertrophic scars on eyelids Long term. I have seen a little bit of have a hypertrophic scars at the inner corners or out of corners and those did respond well to injections of steroids and 5-FU.  The medial epicanthoplasty incision maaaaay need injection but see a oculoplastic surgeon near you first.Chase Lay, MD
