Half my nose tip is deflated, c shaped bridge, saddle depression! What do I do? First nose surgery (Photo)

Anatomically you would be a great patient for rhinoplasty. You should share your concerns and desires for the changes you wish with your rhinoplasty surgeon.The goals of such a procedure might include lowering the bridge, straightening the nose, and refining the tip. The cosmetic changes on the outside are called a rhinoplasty. Your rhinoplasty surgeon will look inside your nose to determine if your septum is straight( if not a septoplasty can help to straighten it). Regarding the enlarged turbinates, your doctor will be able to tell you if an outfracture, submucous resection or partial resection is indicated. I always treat them conservatively as they help to warm, moisten, and filter the air that we breathe.

The mild supratip depression only on the right side breaks the shadow lines of  the nasal dorsum on one view. These can be improved with small crushed cartilaginous grafts or fascial grafts.

You have done an excellent job with the photographs.  There is a depression in the dome area on the right side of the tip which needs to be corrected with a small cartilaginous graft.There is also mild supratip depression only on the right side which gives a break in the nasal dorsum on one view.  These can be tackled very well with small cartilaginous graft placed in tight pockets.  The one word of caution is concerning the shape of the nostrils when looking from below.  Nobody looks at her nose from below and it is not something that needs to be addressed.RegardsDr. J

 The photos presented do not show a saddle deformity. The lower lateral cartilages of the nasal tip are inverted creating asymmetries and irregularities. A rhinoplasty procedure can address smoothing out the tip with a combination of cartilage grafting,cartilage suturing techniques and cartilage removal. The dorsal hump can be shaved down. The nasal bones can be straightened and narrowed with osteotomies. It is very important to have realistic expectations regarding  a rhinoplasty procedure because rhinoplasty and especially the Asymmetrical nasal tip is the most difficult procedure to perform correctly in the entire field of cosmetic surgery.

Thanks for your question and the detailed photographs. You have some irregularities of the nasal tip cartilages that could be corrected by fine adjustment of the alar cartilages using a combination of scoring, trimming, suturing and possibly grafting to create symmetry and balance. I believe that is best done through an open approach. Your dorsum is slightly convex but not too bad. It could be changed but I would suggest only a minor reduction at most. Make sure you convey your concerns and aims very precisely to your surgeon. You should be able to work with your surgeon to generate a digital image of what you want
