2 years post BA tubular breasts. Will my lower pole tighten again if they are removed? (Photo)

Question

My implants have settled after two years and the shape is perfect. However, it’s hard for me to work out like before and I feel Ike they’re too big and I want them removed. I had mild tuberous breasts and my surgeon had to release the tightness on the lower poles of my breasts. Will it tighten again if they are removed? How do I avoid that? I just want normal round shaped breasts without implants.. please help.

Great question!  Nothing is ever a guarantee, but if the contraction was released, it should have matured and healed in already and should not create the tightness again.  However, depending on the size of the implants, I would be more worried that since the skin has been stretched out now, you may end up with deflated breasts with just an explantation and would be unhappy with the appearance.  An option could be downsizing, but you should have a consultation so that your options are reviewed and your goals reviewed.Best of luck and hope this information helps!

it is hard to tell for sure, but if your surgery was done correctly, there is a small chance that your breasts will change to pre-surgery shape. If you are considering a surgery, I would suggest you to consult a board-certified plastic surgeon. Only after a thorough examination you will get more information and recommendations.

In general, the smaller the implant and the less the amount of time it has been since placement, the greater the chance the breasts will return to their preoperative state. Breast implants over time tend to thin tissues, cause a decrease in size, and create more droop. No one can truly predict this as each soft tissue envelope will respond differently, but you may do well with just removal.

It is unlikely that your lower pole will tighten since it has been released and stretched with the implant for the past 2 years.  However, without a thorough exam of the amount of breast tissue in the lower pole and the changes in the skin, it is difficult to determine if you will have a normal round shaped breast after the implant removal.  It is best to be evaluated by a board certified plastic surgeon.  Good luck.Dr. Eric Wright

Unfortunately, you have not provided photographs or told us your bra size.  It is possible to undergo explantation using a periareolar incision and doing a breast lift using a modified Benelli.  At the same time, fat transfers can be performed if additional volume is desired.  This may be an alternative for you to consider.Best Wishes,Gary Horndeski, M.D.

Unlikely the lower pole will 're-tighten'. I recommend an in-office examination as well as a detailed discussion with a surgeon who you are comfortable with.   Finally, make sure your surgeon is a Board Certified Plastic Surgeon certified by the American Board of Plastic Surgery (ABPS).

Likely your surgeon changed or modified your existing breast tissue, including at the lower pole. Also your implants are likely helping with the current shape that you like. The implants can be certainly removed, but likely the shape will change. You could go with smaller implants or have fat transferred to the breasts to restore some volume and shape.

After two years, the skin may tighten a bit, but it is difficult to assess not know the quality of your skin.  It also depends on how big of an implant you have in place now.You can certainly remove the implants, or go smaller.  If you have enough fat, then fat transfer can be a good option to help contour your breasts after the implant removal.  I generally advise my patients to look at the fat transfer as more a contouring procedure than giving you volume.I would recommend you see a board certified plastic surgeon to discuss your options.Hope that helps.

Unfortunately, it is impossible to know whether the lower pole tightness will return. Scarring can be unpredictable. In my experience, if properly released (which sounds like it was given your nice outcome), it shouldn't go back to how tight it was.  It will either be completely resolved or significantly improved. In some cases, a residual amount of memory of that fold comes back after implant removal.  Will need an exam to really know that.
