Thank you for your photo. The nose naturally does not contain a great deal of fat. A "fat" nose is usually due to the underlying cartilage strucutre of the nose as well as the thick skin. Both can be modified with a rhinoplasty to give your nose for a more elegant and defined nose. There is not a significant amount of fat present under the skin of the nose, so techniques like liposuction that work elsewhere in the body won't be useful around the nose. Assuming you are interested in reducing the size of your nose, a surgical rhinoplasty will be the answer. You will want to seek consultation with an experienced surgeon so you can discuss your specific concerns in detail and he or she can give you the best recommendations for your nose and face.In general there is significant swelling and a dressing on the nose for 1 week after surgery. There is still significant swelling for another week. During those first two weeks any bruising that occurs (which can range from mild to more severe) will usually go away. Most patients feel comfortable to go out of the house within 1-2 weeks of surgery. After the first two weeks, you look much better, but there is still a good amount of swelling. After 6 weeks, most of the swelling is gone, but if you pay attention there are still changes which occur after that, although much more slowly. First of all, there is no fat in the nose, the nose is composed of skin, cartilage, and bone. A rhinoplasty procedure can accomplish reduction of the bulbous nasal tip with suture techniques and conservative cartilage removal. Osteotomies are performed to narrow the bridge line. The dorsal hump is shaved down which is usually composed of both bone and cartilage. Anticipate a cast applied across the bridge of the nose for one week, visible bruising and swelling for two weeks, and full healing to take one year. Nasal contour is determined primarily by the underlying bone and cartilage. In some patients thick skin will cause a bulbous appearance to the nose but little fat is present. The skin can subtly be thinned but reduction of the nose is achieved by rhinoplasty surgery. Consult with a physician who specializes in this work.
