Muscle repair revision - is it possible to not have enough skin? (Photos)

Hi! I had a standard tummy tuck Feb 2025 and disappointed in my muscle repair results. I'm seeking out surgeons for a revision consultation but my fear is I don't have excess skin for a second surgery. Is this a valid fear and does a second surgery come with additional risks? Thank you for your guidance.

It appears there is not enough skin to remove at a revision. The muscles can be retightened thru the previous tummy tuck incision and no skin would have to be removed. There are no excessive risks to redoing this procedure.

It is very difficult to give you good advice based on this single photo without in person evaluation. It does not have to be only weakness of the abdominal muscles to have this kind of abdominal protrusion; it is frequently associated with posture, spine deformity, etc. in person consultation with an experienced board certified plastic surgeon or two would help to make right decision. First of all, I’m sorry you’ve been disappointed with your result after going through such a major surgery and recovery. Without an examination it’s very difficult to know why the result feels disappointing. Sometimes there is residual or recurrent muscle laxity, while in other cases the concern is actually related to contour, tissue quality, internal fullness, or scar-related tightness rather than the repair itself.Lack of excess skin does not necessarily mean you cannot have a revision, but it can absolutely make revision planning more challenging depending on how tight the abdomen already is and how much additional muscle repair is needed.Revision tummy tuck surgery is usually more technically demanding than a first-time procedure because the tissues and blood supply have already been altered. Risks can include delayed wound healing, higher tension on the closure, contour irregularities, thicker scars, healing problems around the belly button, and sometimes more limited improvement than patients expect.That said, I would not assume you are automatically “not a candidate.” In revision cases, an in-person examination matters much more than photos alone because tissue mobility and abdominal wall quality are difficult to judge otherwise.An experienced revision surgeon should be able to tell fairly quickly during consultation whether enough tissue mobility remains to safely perform another repair and what type of improvement is realistically achievable.

Yes, this can be a valid concern in some patients, especially after a recent tummy tuck where a significant amount of skin was already removed. However, not having much excess skin does not automatically mean a muscle repair revision is impossible.

In many revision cases, the surgeon can still access and improve the muscle repair, but the surgical approach depends on:

* How tight the skin closure already is
* The quality and laxity of the remaining tissues
* Whether the issue is true muscle separation or residual swelling/fullness
* Your healing and scar characteristics

Sometimes the abdominal contour concern is not actually a failed muscle repair, but rather residual swelling, scar tissue, internal fullness, or persistent skin laxity. Since your surgery was in February 2025, your abdomen may still be evolving depending on your healing process.

Revision surgery does come with additional considerations and risks compared to a primary tummy tuck, including:

* Reduced blood supply to the tissues
* Increased scar tissue
* Higher tension on closure
* Delayed wound healing
* Risk of widened scars or skin healing problems

That said, experienced revision surgeons deal with these situations frequently. In some cases, only limited skin removal is needed. In others, the muscle repair can be revised internally without requiring a large amount of additional skin excision.

You don't need to have extra skin to do a tummy tuck revision. It looks like the problem is not excessive skin, but lack of proper plication of the abdominal wall. The procedure can be done through your existing scar. You need plication of the rectus from the xyphoid to the pubis and plication of the external oblique muscle to the lateral edge of the rectus sheath.
