I had filler done under eyes in 2020, but wanted a permanent solution so a yr later I decided to get lower bleph. On my 4 month f/u I told the dr. I was unhappy with the results of surgery, he said it was due to residual filler from 16 months ago so he dissolved it. Made it worse. Filler should have been dissolved before surgery. Now 6 months since surgery and my eyes look worse than before I had anything done. Still have bags, but the fat that was taken made hollows below my eyelid. Pls help :/

Answer: Residual deformity after fillers in lower eyelid It is very difficult to discuss this issue without proper preoperative photos. It is true that attempt to remove implant material with enzyme may be considered before surgery. However, it is not always successful because implant (filler) is encapsulated with scar tissue which neither enzyme nor topical corticosteroid may resolve. Direct removal of material may be needed through eyelid incision and skin only eyelid flap in some patients. Unfortunately, all of us doing lower eyelid surgery encounter this problem more frequently, in particular when treatment is done multiple times and by "injectors" who are not properly trained to deal with complications. Answer: Circles under eyes after lower blepharoplasty At this point it is best to wait a full year to let inflammation subside and the so fat grafting. It is likely you will need to do it more than once for an optimal result but in an experienced surgeon’s hands it is an office procedure with a very low problem rate.

Answer: I practice aesthetic eyelid surgery revision. Each situation is unique. Under eye fillers are powerful but also extremely hard to do well. I strongly advise you against having grafted fat placed in the under eye and cheek. These are never good and can not be adjusted. The grafted fat causes scaring in planes that need to glide. You likely have retained filler in the under eye. It is hard to remove and often very high doses of enzyme are needed. Regarding "fixing" the problem, this will very much depend on on your issues and this simply is not conveyed in a photograph or a video phone conference. These is help available to make your eyes your best feature.
