Hi. My expanders are very wideset which i assume impacts the little projection. Im worried about a few things but trust my PS that they will be able to correct when the expanders are switched for implants. They are 400cc expanders with 500cc filled. Was hoping to be a lot larger. Can the implants be brought in towards the center on my chest during the exchange surgery? And my second concern is the new pocket is much higher on the second side- can that also be adjusted?

Depends on your skin condition after mastectomy, if your skin is thin but healty and well vascularised after mastectomy, yes prepec reconstruction can be done. But you may need a fat transfer procedure after six months to cover the visible parts of the implant
The separation of the implants is something that can likely be corrected, however an in person physical exam would need to be done to determine exactly what would work best. Fat grafting is likely not the best answer as it will not provide enough bulk in that area to provide the volume you are...
All expanders are teardrop shaped in order to appropriately shape the pocket and preferentially expand the lower pole of the breast. However, almost all permanent implants are round shaped. I would discuss this with your treating surgeon as any teardrop shaped implant comes in a textured form....
