I've had these for 2 1/2 years and they don't fit my lifestyle. Here are my before and after. My muscle was cut in order to put these implants in. Would explanting distort my naturally flat chest? Would my chest be all scarred up? Or is downsizing a better option? I would want significantly smaller.

Sometimes, it is a good idea to remove the implants and give it some time (6 months or so) to see how much the skin contracts. If you feel like you need more volume then you can get small implants. This method really helps with planning and getting the long-term result that the patient wants. Good luck.

Answer: Breast implant removal Based on your photos you can anticipate some loose skin after explant, but the look may be acceptable to you as I don't think it will be severe. One option is to have the implants removed and wait to see how you look after you've healed. If you are unhappy you can consider a small lift or implants at that time.

Removing them would likely leave you looking similar to how they did before, though it will take time for your skin to heal and shrink down after removal, and they won't be *exactly* like they were before. Early after removal, the skin may look loose, though I would not call it distorted. Typically only a small amount of muscle attachments are released for implant placement, so unless there was something more done in your case, this alone would not be expected to lead to a distorted chest. If you have capsular contracture or scarring internally, then you may need a capsulectomy or capsulotomy at the time of surgery. Removing the implants versus downsizing really depends on what your goal is. If you would be happy to have them similar to how they were before implants, then you could opt for removal. If you want to have some volume, just smaller, than downsizing and maybe switching to moderate plus profile instead of high profile is a good option.

Based on your photos and the quality of your skin, I would expect that you would have a nice result after implant removal without a lift. You may consider breast fat grafting at the time of removal, but you have to have an adequate amount of fat to donate to your breasts. I would recommend avoiding a lift and allowing everything to heal for 3-6 months before considering a second stage breast lift. I think that it's unlikely you will need a lift.
