Traditional liposuction awake and smart liposuction are old methods and do very poorly for tightening skin or significant skin tightening or contouring. Small problem areas can be improved slightly by these methods. The best in body contouring not only removes fat with energy devices like VASER ultrasound but also internally heat the skin with radio-frequency devices such as J PLASMA/RENUVION, InMode BodyTite, ThermiRF to heat the under surface of the skin and tighten it at the same time as fat removal. These combinations are called 4D HIDEF VASER contouring and can only be done by a well . trained body contouring specialist. At the same time fat should be harvested, PRP and stem cells added and injected into muscles or indentations/cellulite to get body shaping and contouring. This is known as a Brazilian butt lift when you inject the fat to the buttock for contouring and lifting, but fat can also be used in areas such as the chest/breast, arms/shoulders, calves and face. Cellulite should also be treated with cellfina, sculptra, subcision and/or fillers such as bellafill, renuva synthetic fat, or sculptra. If skin is very loose before surgery then it will need to be removed such as lower abdominal skin (tummy tuck/abdominoplasty), breast skin (breast lift), arm skin (brachioplasty). Overall you need a full assessment, but simple liposuction alone will not tighten skin and needs a combination of various methods -- fat removal, skin tightening, defining, fat grafting, skin removal -- to get the best in body contouring and body sculpting. I would avoid doing liposuction or any elective cosmetic surgery after a PE regardless of whether it is under local anesthesia or a general anesthetic and regardless of whether you are cleared or not.
Regardless, I would require medical clearance from your pulmonologist. You should be monitored during the procedure. If your surgeon is not board certified or operating in an accredited facility, seek another consultation.
You will need to get a medical clearance from your pulmonologist who needs to understand the scope of the surgery from the surgeon and MD Anesthesiologist.
Best to receive written consent from your pulmonary doctor with the understanding under local anesthesia there are no breathing monitors used. I recommend IV sedation with anesthesiologist monitoring you, your risks would preclude me using local anesthesia without monitoring alone. To determine your risk of having a repeat pulmonary embolism and to minimize the chance of this while still balancing controlling bleeding during surgery you will need the input of the hematologist and pulmonologist.Understanding what your risk factors were for your first PE is very important.Having had a PE in the past puts you at increased risk for having one in the future.Personally I’m a big advocate of the week Liposuction and do all my Liposuction including fat transfer cases using local anesthesia with mild sedation. When done properly this makes it a safer procedure. Being awake for Liposuction does not eliminate the risk for blood clots which have been documented with awake Liposuction.Please talk to your doctor who is managing your follow up from the PE.There’s no yes or no answer. Pulmonary Embolism or "PE," where a blood clot travels to the lungs, is a life-threatening condition that requires evaluation and treatment by a medical doctor. It is critical to identify the underlying cause of the clot, to see whether it was caused by a specific event (such as a trauma or prolonged immobilization) or if it was caused by a medical condition that makes your body more likely to clot (a so-called "hypercoagulable condition"). Your medical doctor will help determine the cause and educate you on the need for future treatment. Depending on the cause of the PE, it may mean that you need to take blood-thinning medications called "anticoagulants" (such as Warfarin, Eliquis, Xarelto, or others). Surgery of any kind in patients who are taking anticoagulant medications presents an increased risk of bleeding and bleeding-related complications (such as hematoma or bruising). These risks must be weighed carefully when considering an elective procedure like liposuction. If your medical doctor has prescribed a short course of anticoagulation, it would be best to wait until after you are no longer taking anticoagulants before proceeding. If your medical doctor has prescribed lifetime anticoagulation, it is important for your plastic surgeon to work with your doctor to manage this medication around the time of surgery. It is not impossible to have a surgical procedure while on anticoagulation, but the risks of complications must be carefully weighed against the urgency of the procedure, and this discussion should include both your plastic surgeon and medical doctor.
