Excellent question! 360 lipo alone will make for a smaller waist so it will give you a nicer silhouette, but I believe combining liposuction with BBL would be a better option in your case. Fat grafting to the hips and outer portions of your butt will help accentuate your hip to waist ratio. If your not interested in a bigger butt, the BBL can be performed to improve your butt's shape rather then just increasing it's size.  This will translate to more curves and an even better hourglass shape.Best of luck on yup 360 lipo and BBL,Dr. Miguel Mota

based on tthese photos you may need skin removal but you for sure need lipo 360 and BBL with fat to the buttock and hips. otherwise you will not see much of a contour change. hormonal therapy will be very important as well to tighten you skin.
You need a formal evaluation by an expert in body shaping and contouring to determine what is best.  Traditional liposuction awake and smart liposuction are old methods and do very poorly for tightening skin or significant skin tightening or contouring.  Small problem areas can be improved slightly by these methods.  The best in body contouring not only removes fat with energy devices like VASER ultrasound but also internally heat the skin with radio-frequency devices such as J PLASMA/RENUVION, InMode BodyTite, ThermiRF to heat the under surface of the skin and tighten it at the same time as fat removal.
