I think you would benefit from a lollipop lift with implants. This will elevate and reduce the size of your areolae.,I understand the logic behind the expander because your skin will be tight afterwards. However expanders are very expensive. It makes more sense to place an implant at the time of the lift and replace with larger implant later if needed. either that or do the lift first and implant after about three months. To get the best result you are probably looking at a two stage procedure.

I highly recommend under the muscle to prevent as much direct contact between implant and the breast tissue. This is important in preventing potential capsular contracture risk.

Although your breasts may be elongated in shape, they don’t appear to be truly tuberous. This is because it doesn’t appear that you have any constriction of tissue around your nipples. For a rounder breast shape, you can consider a straightforward breast lift. If you’d like to reduce the size of your areolae, an areola reduction can be performed at the same time.

tuberous breasts are not simply small or underdeveloped breasts. The appearance of the breast can range from mild to severe, and typical traits include: enlarged, puffy areola, rather wide spacing between the breasts, very little breast tissue, sagging, higher than normal inferior breast fold, and narrow base at the chest wall.The type of surgery best for you will depend on the severity of your condition.My preferred technique for tuberous breast correction involves a multistep procedure where the breast tissue is released in a radial pattern to prevent herniation into the areola and placement of a custom implant size is in the breast to allow appropriate expansion of the breast in the restricted dimensions. This can often be done in a single procedure but sometimes may require two surgeries.That said, it is best that you visit a board-certified plastic surgeon for an in person examination. Unfortunately, tuberous breast deformity is very complex. Even if you were to find the top 10 plastic surgeons in the world, their treatment of this problem would differ in their approach, incisions, implants, and placement. It is possible to give you implants with a breast lift and correction of your tuberous breasts in one stage. I would recommend a consultation with a board-certified plastic surgeon who you feel comfortable with. You should also review before and after photos so you know what to expect. Good luck!

You seem to be a good candidate for a single stage breast augmentation using silicone implants under the muscle as well as areola reduction/lift. These procedures are challenging and need to be done expertly. Is best to have a virtual consultation to discuss options of course.

Tuberous breast correction is always challenging but very good results can be achieved. I generally use a combined under the muscle approach for the upper two-thirds of the implant and above the muscle for the lower third which allows the breast to be filled in on the lower portion. I would suggest the use of silicone implants to give a more natural feel. The areolas also have to be reduced. Using this approach can lead to very good looking breasts. Of course, you need to be examined in the office to corroborate the plan .
