Attached is some pictures of my nose as well as some morphs that I would desire out of a rhinoplasty operation - is these morphs possible with twilight anesthetic instead of GA, as well as possible at all? My issue is my nose is just huge in comparison to the skull and I want it a bit smaller so it is more harmonious with my face. Some have said it’s my chin being weak, but my chin is at least average and I’d like to leave that and fix the nose. Is it possible to shrink my nose to fit better?
Answer: Your nose has strong "projection." More ... Projection refers to how far forward away from the face the nose rests. In your morphs, you decreased the projection of the tip, although really, you made somewhat less of a change than I would make in a morph. And I agree, your chin is fine. How *much* to deproject the nose, or make other changes? We figure that out with morphs, well before surgery. I'd love for you to see some excellent professionally-designed morphs of what could possibly be done with your nose. Morphs could also help you identify better just what's bothering you, and help you set a goal for the rhinoplasty that's accurate for your tastes. Profile and three-quarter views would be particularly important in morphing your nose. (Side note: in my opinion, morphs should really be done by the surgeon, or he should direct an assistant as she makes the morphs. Morphs should be made with a constant eye to what actually *can* be done in surgery, for that particular nose, and the surgeon has that information and judgment best.) Finally, remember that rhinoplasty is an exquisitely difficult operation to get right, and you should only have surgery if you are able to make yourself very confident in your surgeon's skills. The changes that your nose needs -- the deprojection -- require advanced techniques, and skill that most plastic surgeons don't possess with expertise. It's much better to not have surgery than to have inexpertly-performed surgery. Read my essay on how to stay out of trouble while selecting a rhinoplasty surgeon. And it also discusses how to take photos that are best for online evaluations.

If you’re motivated enough to have surgery, then start the process by scheduling multiple in person consultations with local plastic surgeons, who seem to have extensive experience with rhinoplasty surgery. Look for providers who been doing rhinoplasty for at least one to two decades. Ideally, you should be in the hands of someone who specializes in rhinoplasty surgery exclusively or at least someone who Rhinoplasty surgery represents a large proportion of the surgeries they do. During each consultation, ask each provider to open up their portfolio and show you their entire collection of before, and after pictures of previous patients who have similar facial characteristics to your own. An experience provider should have no difficulty showing you the before and after pictures of at least 50 previous patients. Take careful notes during each consultation, especially regarding the quantity and quality of before and after pictures. Begin reviewing lots of before and after pictures of Mail rhinoplasty surgery to get an idea of what quality results should look like. Use pictures of your own face for reference when reviewing before and after pictures. Look for other patients who have similar facial and nasal characteristics. Morphing is a useful tool for communicating desired outcomes, but what can be accomplished with a computer is not always the same as what can be accomplished during surgery. Considering the complexity of rhinoplasty surgery it’s probably not a bad idea to have multiple consultations before selecting a provider. Best, Mats Hagstrom, MD
