Hi doctors, I’m seeking professional advice regarding the loose skin and deep creases under my buttocks following major weight loss. I’ve attached a photo to show my current condition. I am 5'4" tall, weigh 109 lbs, and lost 100 pounds about a year ago. I’d really appreciate your expertise on the following: What surgical or non-surgical options are available to remove or significantly improve these wrinkles and creases under my buttocks? Where would the scars be located if surgery is recommended, and how visible are they over time? Is it possible to fully resolve this issue through glute and hamstring hypertrophy in the gym, or is surgery the only effective option? Could you please provide a realistic prognosis of what kind of result I could expect at best—and at worst—with or without surgery? Any before and after photos of patients with a similar body type and condition would be incredibly helpful in setting expectations. My goal is to have a smooth, tight, rounder buttock appearance with no loose folds or wrinkles underneath. I’d love to hear your thoughts on the best way to get there. In order for a fat transfer to be successful, the fat needs to be drafted using good technique into healthy tissue with a good blood supply.
The tissue the fat is grafted into is the host tissue supporting the grafted fat until it establishes a new blood supply.

I generally do not recommend having fat removal below the buttocks on the upper thigh.
It doesn’t usually fix the problem and creates new issues.
If we’re moving fat created the problem than the solution would be restoring the fat with multiple sessions of transfer.
