Would a closed rhinoplasty help target large tip and uneven nostrils? Goal is a more defined and feminine look (photos)

Based on the fact that most of the changes are in the tip and nose base, my option would be open rhinoplasty (we do both) for better accuracy. However, that is decision that you should leave to your surgeon; it is expertise of the surgeon not a technicue, that counts. Recovery, in both cases, should not be significantly different. A small and natural nose is possible via closed approach. İ do mostly closed approach my primary cases and the results are satisfactory

Hello! Thanks for the photos and questions. You can perform different rhinoplasty techniques through a closed approach. Patients often worry about the scar at the columella. If done well, it tends to heal inconspicuously long-term. I find the open approach to be superior due to the ability to fully visualize the underlying structures and be very precise with the changes that should be made. Swelling does vary between the two with the open approach leading to more. Refining the tip through a closed approach is possible. I find nostril asymmetry to be more challenging that way. A closed rhinoplasty also involves scars, it's just that they are confined to inside your nose in the mucosa. While open rhinoplasty does involve a longer recovery time in that the tip remains swollen longer it would be a better approach to improving your nostril asymmetry. Although it is hard to tell without looking inside your nose, your nostril asymmetry may be due to septal deviation.

From your photos, I note that the nose appears midline, the nasal bridge appears to be of normal height and width, the nasal tip dome is minimally blunted. The columella appears to arise from the upper lip at a normal angle. The medial crura of the lower lateral cartilages are somewhat enlarged at the columellar base.
A closed tip rhinoplasty may add a smidgen of definition to the nasal tip.
Visit a board certified plastic surgeon for evaluation and discussion.
