Is Bellafill a safe under eye filler?

Question

I'm seeing and hearing mixed feedback about the use of bellafill under the eyes. I've used Restalyne for under eye treatment once before and was hoping to find a more permanent solution. I was hoping Bellafill could be that solution. My current cosmetic specialist simply said, "Bellafill under the eye is a really really bad idea" but didn't elaborate. Looking for a bit more guidance on the pros and cons. Any semi-perm/perm solutions are appreciated. Many thanks!

Thank you for your question!  Fillers like Belotero, Volbella, and Restylane Silk are good for improving under the eyes, but keep in mind that a combination approach will give the most improvement. My patients try to get non-surgical treatments to avoid surgery, so we use fillers, lasers, microneedling/PRP and stem cells. Dear patient, I have had to remove Bellafill injected too close under the eyes by another surgeon. It is exceedingly difficult to remove Bellafill once the body has had a chance to make Collagen around the small plastic beads. Small irregularities and and bumps are readily visible under the eye. I would agree with your surgeon that Huyaluronic acid fillers are much better suited for the tear trough area. 
In my experience Hyaluronic acid fillers such as Restylane and Juvederm last much longer under the eyes than elsewhere. I have discussed this with other Dermatologist who agree and have noticed the same. The other benefit of hyaluronic acid fillers is that it can be dissolved if Tyndel sign or irregularities occur.

Injections under the eyes for so called "bags" should only be done by very experienced injectors.  Bellafill cannot be reversed and is not indicated for under the eyes.  Personally I like Restylane under the eyes either by injection with a needle or cannula.

Since Bellafill is a long-term solution often lasting 3-5 plus years and is non-reversible, only very experienced injectors are least likely to experience problems with it. Injecting under the eyes is an off label application of Bellafill, but can be a long-term beautiful correction if done by an experienced injector, very cautiously.

Bellafill is a permanent filler and cannot be dissolved. the only way to remove it is via surgical excision, which is difficult to do as it creates an inflammatory reaction with fibrosis and scarring. I would not recommend it for under eye treatments

The most problematic area for Bellafill is in the periocular area. It needs to be placed perfectly for great results. The problem with Bellafill in this area is nodularity and swelling. The area around the eye anatomy changes with aging and this is difficult with a long lasting filler. I have done it in this area judiciously but like to top off with an HA filler. If you notice, most of the questions about Bellafill problems are in this area too.

Bellafill, as a filler, gives natural and long-lasting results.  It is great for the laugh, cheeks, and marionette lines.  It should NOT be used in the tear trough (under eyes) or in the lips.  Both regions have muscles in constant motion and increase the likelihood of lumps forming under the skin.  Fillers that are safe in the tear trough region include Belotero, Volbella, and Restylane products.

I no longer use Bellafill as an under eye filler. The results can be great and permanent. But on the rare occasion that a nodule or granuloma develops, correcting this complication can be a major challenge.  HA fillers or PDO threads are better options for under eye volume loss.
