Hello! I received butt implants and now the skin below the implants is saggy. The surgeon advised that going with a longer implant would put me at high risk for implant displacement or flipping. If I get an infragluteal thigh lift, will the scars be hidden in creases? I haven't been able to find before and after photos for this procedure online. I have very low body fat (no pinchable fat anywhere), so fat transfer is not an option.
Answer: Buttock lift via infragluteal thigh lift Buttock lift/ thigh lift with incisions in the infragluteal crease should help minimize the sagging. The goal is to put the scar in the crease but risks include permanent scars, keloids or raised scars, wide scars and scar migration. Discuss with your surgeon, if off-label fillers could possibly help or small amount of fat grafting.

Answer: Posterior lift The infragluteal thigh lift can result in the scar dehiscent due to it's location and loading from sitting in the flexed position. An alternative to consider is a posterior lift which involves the scars around the buttocks higher up and this can be done to lift up the loose skin but you will have a more visible scar. Best Wishes, Gary Horndeski, M.D.

Answer: Treatment for lower buttock skin laxity Allow me to answer your question as best i can based upon your photos. However, please keep in mind that an in-person, or at least a virtual, consultation with physical exam is first necessary before any legitimate evaluation or final surgical recommendations can be made. The first question is, do you currently have a round or an oval shaped implant? If round, then indeed a longer oval shaped (Stanton Anatomic®️) buttock implant which properly extends the full length of your gluteus muscles will likely fill in some/most of the saggy skin. On the contrary, if these implants are performed correctly, there is no increased risks of flipping or displacement. If still after a Stanton Anatomic®️ implant is placed you have some residual lower buttock laxity/sagging, then a lower buttock tuck may be considered at that time. Please note that an extremely few number of board certified plastic surgeons specialize in buttock implants or lower buttock tucks, so you may need to travel far and away from home to find this particular surgeon. Glad to help.

Answer: The problem with implants The lack of sufficient longitudinal augmentation is only one of many problems with gluteal implants. The list of complications and undesirable side effects of this procedure is longer than any other operation. I think I’ve ever seen. Would your experiencing is only one of many undesirable side effects of the surgery. I wouldn’t have any operation. I couldn’t see an impressive collection of before and after pictures of first. Simply finding a provider who has enough skill with this procedure is going to be difficult. Consider consulting with the guy who first described the thong lift Dr Constantino Meditta. I’m curious how many before and after pictures you saw before deciding to have gluteal implants? Did any of those patients have the same deformity? Fat transfer isn’t really the correct solution, but I do think you have enough fat on your body to harvest fat for a fat transfer if that was to be the correct solution. Individuals who have thin layers of subcutaneous fat just need more areas treated. For limited fat transfer areas, we don’t really need that much fat. In your case, I’m not sure that fat transfer is the solution. Dr. Stanton may respond to your post and recommend you use his custom made implants. I’m not sure if that is going to make all that much difference. The number of plastic surgeons who do gluteal implants well on a consistent basis is pretty small. Most plastic surgeons don’t wanna have anything to do with this procedure. There’s a good reason for that. Best, Mats Hagstrom MD
