I'm a 40 year old mom of two who has experienced a lot of weight fluctuations over the last 15 years. My breasts have lost a lot of volume and I have basically no upper pole fullness. Despite the fact that they sag, I'm more or less happy with the shape and position of my breasts, but do wish to have more fullness as they feel "empty". I'm wondering if I am a candidate for fat transfer without a breast lift given the sagging factor.

Generally speaking, if the nipples are lower than the inframammary line the patient will most likely need a breast lift. If the nipples are at or above the inframammary line we can generally get by with just a breast augmentation depending on the size of the implant the patient wants. I often still using implant when I perform a breast lift because it provides more upper breast fullness after the lift. If you are seriously considering the procedure, I would suggest a consultation with a board-certified plastic surgeon.
