Allow me to answer your question as best i can based upon your photos. However, please keep in mind that an in-person, or at least a virtual, consultation with physical exam is first necessary before any legitimate evaluation or final surgical recommendations can be made. There are currently only two FDA approved surgical options for augmenting the buttock/hip area: 1) Buttock and cosmetic Hip Implants (which are soft semi-solid silicone rubber implants that cannot rupture &/or leak) and 2) Brazilian Butt Lift (aka BBL) - which utilizes the patients own fat from liposuction then transfers this fat into the buttock. On the other hand, BBL using Sculptra is a dangerous, guarded waste of money, and is NOT FDA approved for these very reasons. So what it comes down to, like any surgery, is proper patient selection and long-term results. Implants provide the only guarantee for a sustained permanent result and tend to be ideal for perking up not only lean or deflated buttocks but also providing substantial projection to any butt, which is what every single patient wants. Unfortunately, you are simply way too lean for a BBL. Beware, if a surgeon asks you to purposely gain weight (i.e. fat) before BBL, know that the fat you lose first as you get back to your baseline weight after surgery is that very same fat that was transferred into your buttock, hence your new buttock shrinks first, the fastest, and the most...so please do not fall victim to this recommendation. Unlike breast implants, silicone gel buttock implants are not FDA approved and thus not legal for buttock implantation in the USA. I actually see about one patient a week that had gel implants done in another country and, although they looked and felt great at first, they are now seeking revision because the implants are either visible on top of the muscle and/or grossly deformed by flipping over, rotating, scar contracture and/or the implant has ruptured several years after placement leaving behind hardened scar tissue my migration of silicone liquid. The soft semi-solid silicone rubber implants used in the USA are much more form-stable and thus much less likely to have these problems if placed correctly within the gluteus muscle AND are considered permanent implants because they cannot rupture and leak like silicone gel implants. In 2015 I created and patented a new Stanton Anatomic®️ shaped implant that avoids the unnatural effects of round implants, whether silicone gel or rubber. The benefit of this is the fact that ~95% of patients, like yourself, have much more of an oval shaped buttock cheek not round, thus a true oval implant (i.e. Stanton Anatomic®️ implant) creates a much more natural and harmonious buttock contour. This is especially true when the patients bends over, squats, or pokes their butt out while striking a pose or dancing. In your case, you would likely benefit from having two slightly different sized Stanton Anatomic®️ implants placed. Regarding the banana roll phenomenon, you can expect it to improve some but never be completely eliminated. Beware also, if a surgeon recommends liposuction of the banana role because this often times, makes it worse not better. Please note that extremely few number of board certified plastic surgeons specialize in buttock implants so you may have to travel far and away from home to consult with this particular type of surgeon.
