It is impossible to see the entire deformity on your photo, unfortunately. It appears that best solution would be direct excision (removal) of involved areas of scars and dimples, placing the scars in glutal crease (between buttocks) and just above the buttocks. Some of the tissue can be used for further shaping/augmentation of the butts. For this type of surgery you may need a true expert and surgery in 1-3 stages!. In person or teleconference consultation with a board certified plastic surgeon with good reputation in body sculpting, is recommended. Hope this helps. Good luck.

Allow me to answer your question as best i can based upon your photos. However, please keep in mind that an in-person, or at least a virtual, consultation with physical exam is first necessary before any legitimate evaluation or final surgical recommendations can be made.

Based upon the photos, you have a complex problem with deep scarring. This will likely require a combination of procedures to fix/improve. My guess is that the deep scarring will have to be exercised and good tissue on either side brought together. In my experience, additional filling out of the depressions will only benefit from buttock implants. Note, due to the extensive scarring, I highly doubt that BBL fat transfer will be effective in your case. Please also note that an extremely few number of board certified plastic surgeons specialize in buttock or hip implants, so you may need to travel far and away from home to find this particular surgeon. It would be important to know what type of injections you had. It looks like you’ve lost subcutaneous fat and less likely there is significant scar tissue as well. This is going to be difficult to correct. I suggest having multiple in person consultations with local board-certified plastic surgeons. Some improvements may be possible but we need more information to make any kind of quality assessment. That includes quality pictures and more information about your history.
