Vaser Liposuction and Renuvion. What causes adhesions and lumpiness? Can this be fixed? (Photos)

I had Vaser Liposuction with Renuvion six months ago. 250 ml of fat removed. What is your assessment of what went wrong? The wrinkles are adhered and will not smooth out even with massage.

Pending a formal evaluation, you may be a good candidate for a Revision Liposuction. You need a formal evaluation by an expert in body shaping and contouring to determine what is best.
The best in body contouring not only removes fat with energy devices like VASER ultrasound but also internally heats the skin with radiofrequency devices such as Renuvion (J-Plasma), InMode BodyTite, and ThermiRF to heat the undersurface of the skin and tighten it at the same time as fat removal. These combinations are called 4D HI-DEF VASER Liposuction and can only be done by a well-trained body contouring specialist. At the same time, fat should be harvested and purified, with PRP and stem cells added, and then injected into muscles, indentations and cellulite to get body shaping and contouring. For a Brazilian butt lift (BBL), we inject the fat into the buttock for contouring and lifting, but fat can also be used in areas such as the chest/breast, arms/shoulders, calves and face. Cellulite should also be treated with a combination of Cellfina, subcision, and/or fillers such as Bellafill, Renuva synthetic fat, or Sculptra. If skin is very loose before surgery, then it will need to be removed: tummy tuck/abdominoplasty for lower abdominal skin; breast lift for excess breast skin; and/or a brachioplasty for loose arm skin. Overall, you need a full assessment, but simple liposuction alone will not tighten skin and needs a combination of various methods -- fat removal, skin tightening, defining, fat grafting, skin removal -- to get the best in body contouring and body sculpting. The worst outcomes from liposuction are almost always a combination of Poor technique combined with over treatment.  VAS ER can remove fat aggressively. This may seem like a good idea but when it's not done with technical expertise outcomes like yours or worse are the outcome.  Unfortunately the more someone has been over treated the more difficult and often is not possible it is to correct the results or do revisions successfully.  The fat is simply removed excessively and scar tissue is left in place. When there is no fat remaining then skin adheres to muscle causing adhesions.  For people with skin laxity converting this to a tummy tuck may be possible but the scar tissue makes the skin and fat layer less pliable making this option more challenging.  As you have a learned massage does not change the outcome is after liposuction.  I'm not sure why so many providers recommend it but it's a waste of money in my opinion.  Massage for the sake of massage is obviously a different topic.  The bottom line is you were over treated. On top of this they attempted etching or excessive accentuation of muscle borders.  I don't believe etching is a good technique. It never looks natural and can cause devastating disfigurement.  It is also more or less impossible to correct when done poorly.  Best, Mats Hagstrom MD
