Question

Procedure done: Jan 2019 My doctor in Turkey made my ab etching asymmetrical and unnatural looking. I also have lots of fluid retention which is noticeable from the side. He also took too much fat out too superficial to the top skin and so you see lots of rippling. Can I fix this with Sculptra or do I need a fat transfer? I personally do not want to do the fat transfer option because I do not want any more loose skin on the rest of my body (already have done a BBL in 12 areas).

I feel bad for you and I really want to be helpful, but you have to understand that it is always harder to take care of a complication than to do the right procedure to start with. You need to be seen with pre and post op pictures in hand and all your medical and surgical history. Depending on how you looked before, your operation could be considered a terrible result or a favorable change. I would suggest to wait a minimum of one year. Then you might be able to get some improvement wit some additional liposuction and fat injection to reduce the sharp contrast between the high and low areas. A very gifted plastic surgeon could plan a procedure taking from the high areas with a fine cannula and injecting in the low areas to smoothen the results. Very few plastic surgeons want to take on a secondary case like yours. Therefore you would have to show low expectation and ask for some improvement, so that someone would accept to take on the responsibility to help you. I wish you the best of luck.

you're right. there is not enough room for fat to be taken out to fix the problems you're having. with that said, there are other ways to fix the issues. they are not the most ideal methods. but they are fairly acceptable. I specialize in fixing defects and revising surgeries. Im often faced with a situation where the patient doesn't have a lot of fat but has a lot of defects. Pending a formal evaluation, you may be a good candidate for a Revision Liposuction. You need a formal evaluation by an expert in body shaping and contouring to determine what is best. 
The best in body contouring not only removes fat with energy devices like VASER ultrasound but also internally heats the skin with radiofrequency devices such as Renuvion (J-Plasma), InMode BodyTite, and ThermiRF to heat the undersurface of the skin and tighten it at the same time as fat removal. These combinations are called 4D HI-DEF VASER Liposuction and can only be done by a well-trained body contouring specialist. At the same time, fat should be harvested and purified, with PRP and stem cells added, and then injected into muscles, indentations and cellulite to get body shaping and contouring. For a Brazilian butt lift (BBL), we inject the fat into the buttock for contouring and lifting, but fat can also be used in areas such as the chest/breast, arms/shoulders, calves and face. Cellulite should also be treated with a combination of Cellfina, subcision, and/or fillers such as Bellafill, Renuva synthetic fat, or Sculptra. If skin is very loose before surgery, then it will need to be removed: tummy tuck/abdominoplasty for lower abdominal skin; breast lift for excess breast skin; and/or a brachioplasty for loose arm skin. Overall, you need a full assessment, but simple liposuction alone will not tighten skin and needs a combination of various methods -- fat removal, skin tightening, defining, fat grafting, skin removal -- to get the best in body contouring and body sculpting. It is hard to tell for sure without an examination but based on your photos, it looks like you have some loose skin in belly button area and some uneven areas after liposuction. You can wait some more time to see the final results of your surgery and to see if this will improve. I understand your concern. However, without a proper assessment it would be difficult to determine what is wrong. It is best that you visit your plastic surgeon for further assessment. Only after thorough examination you can get proper recommendations and advice.

The volume of filler you would require would be completely cost prohibitive and would require repetition. Also injecting in an area of fluid retention is problematic. You should wait this out more to determine if the fluid reabsorbs or requires intervention.

I assume 4K means 4 liters of lipo? Since you do not have any donor fat available your only options are the non invasive injections of Sculptra &/or Renuva in multiple sessions. Both are quite expensive per vial, syringe, mixed cc. Best to find injecting MD, who has done these injections to correct overly aggressive liposuction. BTW the re occurring fluids due to the heat energy of over done VASER lipo.

there's no way to correct previously done poorly done liposuction.  This becomes especially true for people have been over treated.  Catching is by definition aggressively over treating certain areas.  There's no longer post tissue to support fat graft so this becomes a non-viable issue.  Removing all of the fat and the remanding surrounding area's will leave you grossly over treated on your entire abdomen so this becomes a non-issue as well. Sorry to be blunt but you will carry this result for the rest of your life. I do not offer etching and think it's a bad procedure. Overly aggressive liposuction to try to make fat look like muscles looks like what it is.  It looks like inappropriately performed liposuction with fat that bulges.  Best, Mats Hagstrom MD
