I am 58, 5'4", 118 pds. 22 years ago I had gummy bear implants to go from an 32A cup to a C cup. My weight has remained fairly constant but I am now 32D cup with back/neck/shoulder issues. I am looking at getting current implants removed and do not wish to return to an A cup. Am looking at a lift and smaller implants. What is your implant size/ suggestion (ie cc's and profile) for me to go to a healthy B cup? Thank you
Answer: Size You did not state what size you have, but you could consider something between 150-200, maybe 250 cc's for a nice B. Your own tissue has been thinned and stretched with the larger implant.

Answer: Explant with Bellesoma Method Your implants took you from a 32 A to a 32 C so they would be approximately 200 cc implants. The technique I recommend is explantation and lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. But the decision will always start and end with the patient. When it comes to sizing, we have all of our patients find 3 photos of their desired breast size. We ask for naked breast photos because clothing can obscure the true size (push up bras). Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements.Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match.Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process. The smallest perceptible change is roughly 10-15%, which may surprise you. For example, if your implants are 300cc, then you will need to reduce your implant size by 30-45 cc to appreciate the downsizing. With regards to pain, it is unusual to have the common problems that women with naturally big breasts have. If you went from a C to a D, then your breasts grew. No one can perform a surgery to get you to a specific cup, and so that discussion is fruitless. Your issues are medical and possibly cosmetic. I suggest being evaluated for a possible reduction and lift. If your breast tissue is significant, then you may not need to touch your implants at all.
