I am in the consultations phase of my tummy tuck. For background I am 32, female, 5'1, 153lbs with two previous c-sections. At my consultation the plastic surgeon said that I may have a bulge in my upper abdomen area as she cannot agressively liposuction that area due to necrosis. I wanted to ask other surgeons what my options are in regards to this and will it look "that bad?" ***I wish I had asked to see pictures of what she meant but I forgot. Answer: Upper abdominal concerns and tummy tuck Hello,Based on your photos and the health history you provided you are an excellent candidate for tummy tuck.Don't expect to have a bulge in the upper abdomen after your tummy tuck - once the muscles are tightened and the excess skin removed and pulled tight, I expect your tummy will be nice and flat, you really are a good candidate.Your surgeon is correct about overdoing liposuction in the upper abdomen at the time of tummy tuck - it can interfere with blood supply to the wound edges and lead to necrosis, but only if it is overdone. You may benefit from a touch of liposuction in the upper abdominal area, and some liposuction of the flanks, but you certainly don't need much liposuction, the tightening will take care of your concerns. There is no reason for you to have anything other then great results because you are actually a textbook patient for a tt. I do not agree that you may have a bulge even after surgery. Muscle repair plus the stretching of the skin all the way down to the pubic area (area where the scar will eventually be), will be more the enough to flatten your entire abdomen. Excessive abdominal lipo during a tummy tuck may increase the risk of necrosis, but you do not need excessive abdominal lipo. A little bit of lipo in specific areas may be performed to enhance results, though it might not even be necessary. In any case, this will not increase risk of necrosis and you should have great results regardless. No need to worry. A tt will only make you look better.Best of luck,Dr. Miguel Mota

The TT also creates a more youthful belly button and removes the c-section scar. All stretch marks will not be removed. TT without muscle repair may make the recovery easier, but, if you have laxity in that area from any pregnancies, the result will be better with plication. In general, the muscles should be brought to anatomic norms. The smaller the separation, the less tightening required and probably the less discomfort. Swelling will occur irrespective of muscle repair, and almost every patient requiring a TT will benefit from the muscle repair.
