I have a lot of upper pole and no lower pole fullness, breast seem to have never dropped, implants are also very small seems to be around a B cup small C. Silicon gel implants done in 2009 when I was 18 yrs old in Mexicali Mexico I am currently 22.I would like to have more fullness at the bottom of breast and more centered nipples, breast implants feel way too firm. Would it be best to try saline? Currently living in Palm Springs CA

Answer: Too much upper pole fullness Your problem can be fixed by appropriate manipulation of the space and expanding the lower pole. Proper implant selection will also lead to a more pleasing breast appearance.Seek out a board certified plastic surgeon to help you. All the best.

Answer: Correction Of Upper Pole Breast Fullness After Augmentation Correction Of Upper Pole Breast Fullness After AugmentationAs you would like to have larger breasts, I would consider capsule correction and changing to a shaped implant, I use Sientra, that is form stable and will stretch the lower pole to provide better shape, size and fullness.

Answer: Capsule contracture Dear Sbanuelos, Thank you for submitting your pictures. The answer to your questions is in your story and pictures. It is clear that you developed severe capsule contracture. The solution to it depends on the physical examination and it ranges from simple capsulotomy to more complex capsulectomy and replacement of the implants. The later is due to biofilm that has to be completely eliminated. As far as replacing the silicone implants with saline implants, it is probably a good idea if you are going to have larger implants . Larger silicone implant will require longer incisions than the ones that you already have. Saline implants can be inserted through a small incision under the areola which also makes the surgery on the capsule much easier. Consult with experienced board certfied plastic surgeons and check their 'redo' before and after pictures to make sure that you like the results. Best of luck, Dr Widder
