The nasolabial angle,where the columella attaches to the philtrum, is majorly affected by the underlying nasal spine. This usually pushes the angle out. If the spine is reduced, the angle may look better but the length from the angle to the lip may appear longer.

Saline drops will not be contributing to dryness. Dryness after surgery usually comes from resection of healthy nasal epithelium. I would consider seeing one or two other doctors for additional recommendations.

You really have to see a surgeon who can meticulously examine your scars and determine if a revision will help. Scars do not disappear, but can be made better sometimes. Good luck.
