You want to wear whatever underwear is comfortable and doesn't "cut into" your skin or swelling and leave divots or depressions. This is to help make sure the skin and tissues are smooth as they heal. This is most important for the first 2-3 weeks. A thong could be fine as long as it fits comfortably, so you may have to wear a larger one for a short while after surgery.
I usually have patients wear a compression garment for 3-6 weeks. Underwear over that is fine for my patients and it could be anything that they like.

This is a common finding in many patients (men as well as women). if the pockets of fat are small and isolated I would recommend Liposuction under local anesthesia. if there is accompanying skin laxity as well I would recommend mastopexy.
