Nose too wide & stretches out when I smile, nostrils large. My main aim wuld b 2 reduce nose size. I'm unbothered by nose height/tip sharpness, I wuldnt want any implant/procedure to make it so. Doc proposed ear cartilage, but Im not comfortable w inserting additional things. I'd prefer somethg more simple. I've read abt diff techniques of alarplasty. Can it reduce fleshiness of nose if incision is made at cheek-nose line, tgt w reducing width by base incision? Is removing excess flesh on nose possible?

You are correct in that cinch sutures may change your smile. However, it could improve with time and may not need to be removed. I would seek the advice and consultation of your rhinoplasty surgeon.
The injections will be painful, but the surgery will not. Find a plastic surgeon with ELITE credentials who performs hundreds of rhinoplasties each year. Then look at the plastic surgeon's website before and after photo galleries to get a sense of who can...
An alar plasty done as a stand-alone procedure is certainly acceptable to perform. It will not make the tip or bridge appear wider in this case. This surgery can be done under local anesthesia as an outpatient procedure.
