Cosmetic surgery with history of DVT/PE

A history of a blood clot increases risk of recurrence. Your weight loss also decreases your risk. You may consider staging your surgeries to decrease risk. Lovenox along with other measures to decrease the risk of blood clots is also important. 

Yes, you will get differing answers because there is medical literature that is very clear on DVT risk and prevention and then there is your unique situation. I talk about this frequently with my weight-loss support group. I'll give you the medical studies and then I'll give you my OPINION. First, there is something called the Caprini score. This determines if you need Heparin or Lovenox. The answer is YES. Now, I give every single tummy tuck and body contouring patient Lovenox because they are at slightly higher risk due to weight, procedure length and simple statistics on procedures. Many surgeons are afraid of this drug causing bleeding, but I am not. Studies have shown time and again that it does NOT cause increased bleeding. Your risk of hematoma is the same with or without it. Studies also show that the LONGER your procedure the higher the risk of DVT. Those are the studies... My opinion is to protect YOU. Yes, you can get the procedures, but I would stage them to keep each anesthesia/surgery time short and lower your risk. Sure you had a DVT last time, but this time it could be a massive PE which could take your life. If I were your surgeon I would do them separate, give you Lovenox in the pre-op area and keep you on Lovenox for 2 week after. The latest evidence in medical studies is coming out with results showing patients who were on Lovenox for a few days still having clots 1-2 weeks later. These are orthopedic studies, but they are telling. Hope this helps.
