1 year post op - Do I have alar rim retraction from my Asian rhinoplasty? (Photos)

Question

One year ago, I had an Asian rhinoplasty and Alarplasty in Seoul, South Korea. Now, I think I have too much nostril show from my profile view and I have slight triangular notching at the top of my nostrils from the frontal view. Can alar rim contour grafts and/or fascia alar grafts help bring down the rims of my nostrils so that I don't look like I'm flaring my nose all the time? My surgeon says the grafts would only minorly reduce the retraction and doesn't recommend I have additional surgery.

Did you have an L-shaped silicone implant placed? The best option for a revision, and whether you should pursue a revision, depends on what was done to your nose and your aesthetic goals moving forward. Likely a combination of techniques, including the use of integrated alar rim grafts, will provide the reduction in alar flaring that you seek. Based on your photos, your tip projection does appear to be slightly deficient relative to your supratip and dorsal height.

Hello and thank you for your question. Based on your photographs, you may benefit from a revision rhinoplasty. Maneuvers for your surgery would include release of scar tissue, tensioning of the tip, placement of extended alar contour grafts to correct alar retraction, among other maneuvers. With good surgical technique, you could have an outstanding result. I recommend that you seek consultation with a qualified board-certified rhinoplasty surgeon who can evaluate you in person.
