I am 9 months post op and I’ve noticed my tip has dropped significantly and I feel like the bottom of my nose is rounded and bulbous which was not my initial result. I am happy with the straightness of my bridge and did not ask for an upturned tip however feel that the tip is starting to look droopy and elongate my nose. Could this be swelling? Is this something that would need to be addressed with revision surgery?

Answer: Multiple factors It could still be swelling, as the infratip is the last area to lose swelling after an open rhinoplasty, but overall you should be improving now. Give it a full year for swelling to resolve for a primary rhinoplasty, and be seen by a rhinoplasty surgeon who can examine to see if this is swelling or the shape of your nose. If it is not swelling and the shape of your cartilages after your initial rhinoplasty, then revision rhinoplasty would be the solution to reshape the tip.

You may need a minor revision tuck-up of the lower part of the nose to balance out the nostril and columellar proportions on the side view. It would be nice to see the front views as well.

Because of gravity, swelling centers itself on the tip of the nose which can result in dropping off the tip. If the current appearance of your nose does not make you happy, you may consider having fillers on the area to lift the tip. Seems like your concern doesn't require to have another surgery at the moment. You may have the injections first and then consider a revision rhinoplasty if you are still unsatisfied with the result to have a permanent solution since fillers dissolve usually after a year.
